Though she is having satisfactory blood sugars, her blood pressure is high and she is having cardiac symptoms.
Breathlessness and chest pain which responded to nitroglycerin also suggest cardiac symptoms. Continue nitroglycerin till you consult cardiologist for complete cardiac evaluation such as ecg, echo and angiography (if suggested by your cardiologist)

Pin pain after stunting is usually not due to heart. It is usually muscular in origin. Still, I would advise you to get an ecg done, and show it to cardiologist.

Sit in hot water (temprature same as you use for bath) twice daily.
Apply white vaseline to anus after sitting in hot water.
Cremaffin liquid 2 tsf before sleep for 15 days
good amount of salad and fruit and water daily.

Tab nitrocontin is not ideally for blood pressure and it is given for cardiac patients who have angina / blocks in heart vessels and this tab is known to cause headaches so you can safely avoid nitrocontin if you do not have any cardiac problem!

"As" per your clinical history is concerned you have mentioned you have heart disease but not mentioned which type it is and what is the exact name and what are ECG findings.
