Due to his increased Creatinine, he required modifications in treatment. Trajenta is safe for kidney patients. That's why it was added along with decreasing dose of metformin, which was higher in glimser forte 2. Metformin is excellent drug for sugar control that's why he might feel with it, his sugars we're better, But seeing his age and Creatinine status, higher dose of metformin is harmful in longer duration.

Fasting glucose 300 mg is too high. In this situation only Glimiperide 1 mg (Amaryl 1) is unlikely to be adequate. Even the other tablet Zoryl 1 mg is same. His treatment needs to be upgraded right away, by adding some appropriate drugs. Since there is no mention of any other relevant details it is difficult to advise.

Glynase usually works during the first few years & then starts failing. You should consider starting gliptins along with metformin.

Sir you meet your Diabetologist / Consultant who is treating you and report your problem. You shall have to exclude any Neuropathy or Myopathy which is a complication of Diabetes. Please do not ignore.

Mr. Lybrate-user, to reduce blood sugar levels, apart from appropriate medication, exercise and diet control are equally important. So you have exercise daily for at least 1 hour, brisk walk is a good option. Plus diet should be restricted to give exact calories based on sugar levels, daily exercise and ideal body weight.
