Mr aashish, you have mentioned" average glucose level" of 140 - 145 mg, what exactly you mean by that? is this fasting or after food level? please give me current fasting and pp sugar levels and hba1c%, also the level of physical activity done by you daily.

You add glyciphage 1 gm twice daily along with what you take now and let me know
it is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (self-monitoring of blood glucose? smbg). Though difficult, this can be done by a glucometer and keep a record.

WHen in very good control glucose should be: FBG: < 100 mg, PP 150 to 160 mg & HbA1c%, <6.8 or even less. So please look at these values in relation to your glucose levels. That will give a clear idea. Thanks.

you can follow the advice below re home monitoring till the situation is betterand you can meet your doctor
monitor blood sugars with glucometer as follows. Fasting and pp on wednesday and 2 hour after lunch and after dinner on sunday and maintain a record. Target fasting < 120 and pp < 160

Type 2 diabetes is one in which there is normal secretion of insulin from pancreas but there is resistance in its peripheral action.
In type 1 diabetes there is reduced or no secretion of insulin from pancreas hemce they require injection insulin for treatment.

I have seen the details given, With Kidney transplant it is better to avoid Metformin. Please let me know your serum creatinine and urea as there is a need to look at eGFR. Please tell me whether you are on only Actrapid insulin or is there another basal insulin added? Because in your case it is necessary to control blood glucose with Insulins only (Basal & rapid acting). Present glucose level is too high.
