As she is 60 years old, she need to be evaluated very carefully. You should visit a diabetologist or a endocrinologist nearest at earliest, as breathing difficulties is not normal. Her cholesterol level as well as ecg or echo to be done yo evaluate her heart. And blood investigation for her blood sugar levels. And definitely treatment need to be changed.

Ir is immediate release and sr is sustained release formulations. Both work well but usually sr formulations have lesser gastrointestinal side effects.

Hello, thanks for the query.
Both the products are same. The er refers to extended release, which means the drug is released slowly over 12 hours in the gut. That helps maintain a longer therapeutic level. It is beneficial more so if a person has any side effects like gastrointestinal disturbances if plain metformin is taken. Both are fine. Thanks.

Well with diabetes with long duration and with hba1c of 7.5 is good. But still the more you approach to make hba1c < 7, the pp blood sugar is responsible for hba1c in between 7-8 %. And thats what we are seeing, you pp is raised.
My suggestion is take ppg 0.3 or stabose 0.3 thrice a day, immediately after meals.
