So wat you need to do is. Eat more fibers in the evening. And use powder lactifibre with milk or syp. Lactulose with normal water at night before bed.

You can increase the dose of all medicines you are taking but in consultation with your physician.

Since your you have long standing diabetes and adding on acarbose my not achieve the target in my opinion basal insulin will help you more then acarbose.

You can add galvus to your current medicines instead of switching if your liver n kidney function test are normal and if you never had pancreatitis. Monitor your sugar levels every week to 10 days to see response.

I have read the details given. The fasting glucose & HbA1c% are quite high. Therefore, Insulin might have been advised. Plus already there are multiple oral drugs like Glimiperide, Metformin, Acarbose, Vildagliptin & Pioglitazone. WHich means the whole oral range is there.
