Mr metformin can be taken up to 2500 mg per day. But 100 mg is also a high dose only. If it is not causing any gastro intestinal side effects then no problem. It is really good that you have started on insulin because looking at amount of drugs that you are taking, it is necessary for you to shift to insulin. By doing self monitoring of blood sugar daily you can increase the insulin dosage every alternate day till you reach target fasting level. It is good to reduce or take away pioglitazone.

Mr. M s sodhi, you have mentioned fasting sugar level as 130 - 180 mg, which appears pretty high. What is the recent pp level? hba1c% looks good. When does the sugar level go down is there a specific pattern or time of the day? your lipid profile details are not given fully. Important ones are total cholesterol, hdl, ldl and triglyceride, you have not mentioned those. Tsh level is also high, it should be below 5 mill units / ml. I can understand your concern to your Father for the better evaluation please do the some test like HbA1c, Lipid Profile, FBS, PPBS, Insulin and come back to us with full history of the medical treatment, then we can do some modification in the present treatment to control the diabetes.

You seem to be having a fungal infection because of high sugar. It will keep on recurring unless your sugar level is controlled. If you are following dietary restrictions well and still the diabetes level is so high, your medicines will need to be changed. Their dose needs to be increased.

Its very nice that you have controlled your sugar level to normal values,
for your peripheral neuropathy need to take vit b complex inj or tab for your leg cramps need to go for Doppler study.
