Such resistant bacteria in a transplant patient is not a good sign.
Is he having any difficulty in passing urine or his urine is getting blocked?
He will require further evaluation, such as ultrasound of transplant kidney, urodlowmetry and if required few other test
may require reduction in his immunosuppressive drugs also.

I don't understand why you should have this Colony count even after 2 months of treatment. Either the treat is not being taken fully or there is some other reason. This count means a full blown infection. Get a USG done of kidney and urinary bladder to rule out any other pathology and a blood sugar test done to rule out diabetes.
Do not take it lightly. Take a lot of fluids orally and contact me.

Not serious. This indicates UTI and needs proper treatment. Do 1.Pass urine whenever you feel a desire, empty bladder completely.
2.Wipe from front to back after toilet
3.Take lots of water so as to have clear urine
4.Do not use any spray, scented bath products
5.Try to take shower rather than bath
6.Clean genitals before sex and pass urine after sex to flush out any bacteria
7.Keep genital area dry- wear cotton, loose under garments, avoid tight clothes like jeans
