Its highly unlikely that a psychiatrist will start anyone with anxiety disorder on lamotrigine and gabapentin. There are other better and safe medicines available in the market. Anyway, for now you should consult a psychiatrist for reassessment and to decide whether these meds can be tapered off and stopped.

B12 deficiency can cause tingling sensations and occasional numbness in extremities. Your vitamin d levels are marginally low.
You need to take injectable preparation of b12.
Initially it would be daily im injections for a week, then once weekly for 4 weeks and then once monthly on regular basis.
Gabapentin is a palliative medication which can temporarily lessen your neuroathuc pains.

consult your physician regarding continuation of gabapentin. If you're feeling better with tablets then start with back stretching exercises and strengthening exercises. Start walking daily for 20 min. Change your lifestyle like avoid sitting for long time, avoid bending forward lifting heavy weights. Keep check on your body weight.
