There may be multiple reasons for current problem.
We need more information like any symptoms of hypoglycemia like giddiness, sweating, increased hunger; diet pattern, any associated kidney, heart,liver problem; current hba1c.
You have not even mentioned regarding how many tablets he is taking daily.
Based on limited information provided I may suggest to reduce tablet and get hba1c and creatinine and consult doctor.

you are very right about normal range of tsh levels. At 8.5 milli units/ml, with normal t4, the condition is classified as sub- clinical hypothyroidism. This normally is not treated with any drugs. The information is very inadequate. Please give following details: fasting, PP glucose levels, HbA1c5, creatinine and urea levels. I need to look at his e GFR as he is of 85 years. Then only I will be able to advise. Thanks.

The dosage of various medicines for glucose control can be adjusted on the basis of patients needs, doctor's advise and also based on dietary intake. Usually Glimepiride is either given once a day or twice in some cases. Metformin can be given 3 times a day again depending upon the need. Voglibose is usually recommended with each meal, that is three times a day. As it is mainly for reducing post food glucose rise. Thanks.
