Sir, I am 23 years male. My height is 5'11" & weight is 128. For last 2 years I am having hypertension with anxiety. In last year I had panic attacks two or three times due to anxiety. Now I am taking these following medicines: 1-selomax 50/5 (metoprolol succinate extended relese& amlodipine besilate tablets) 2-paroxee cr 25 (paroxetine hcl controlled release tab) 3-winbp ct40/6.25 (omlesartan medoxomil 40mg &chlorthalidone 6.25mg tab) 4-febumac 40 (febuxostat tab 40mg) 5-bio-d3 max (calcitriol, omega-3-fatty acids (epa&dha, methylcobalamin, folic acid, boron, calcium carbonate soft geltincap) in 1st june 2015 I had bp-150/80, pulse-76& triglyceride-223mg/dl. Sir please advice me about my medication and can I use medication or any type of supplements for weight loss?

I am 26 I have mild sprain in my knee so consult a doctor and given retoz -90, febuxostat 40 mg, tendocel for 15 days And consulted doctor again he told to stop all except tendocel and given rosiflex trio for another 30 days I have used fir another 9 days .after 9 days I felt some no hungry, weakness, for two days so I stopped using those tablets for two days and my stomach felt burning and webt to a hospital and they given me injection and I became dizzy and after few minutes I am okay and after few days I consulted a clinic near me a he asked to take some tests and the result is jaundice 1.86 ,ultra sound fatty liver, vitamin d less i.e 10.8 and recommended to control diet and given liv 52. I have used it then after that my stool came black and one day blood came in my stool and next day it was normal and then gone for tests again and and got my bilirubin level decreased by o.1 and and stool tests was normal but in abdomen ultra sound it showed liver enlarged grade 1 hepatomegaly. What is the condition of my cause tomorrow going for gastro specialist.

I am a 40 years old male, weight: 51 kgs (ht. 5 ft 3 in). I am having high blood pressure since last 03 years. Presently under medication of cilnidipine 10 mg + nebivolol 2.5 mg once daily and my blood pressure is normal. But my serum uric acid often gets elevated for which I take febuxostat 40 mg. My cbc, lipid profile, liver panel, serum na, k values, fasting & pp bs, serum creatinine are all in normal range. Ecg impression is also quite normal, though 2d-echo shows mild left ventricular hypertrophy with diastolic dysfunction. My physician is advising me to switch to losartan 50 mg + cilnidipine 10 mg. What should be the best treatment for diastolic dysfunction/lvh? Should I switch to losartan-cilnidipine combination?

Yes you can switch to losartan and cilnidipine.
Losartan has benefits in reducing uric acid also. It will control bp and prevent remodeling of heart too. Adding cildnipine will further help to control bp by dilating blood vessels and reducing work load on heart.
The mainstay of treatment for lvh with diastolic dysfunction is ace/arbs, diuretics, ccbs and beta blockers!
beta blockers may later be added if needed!

Hello doctor, I am 23 years old male and around 8 months ago, in my blood test I found high levels of creatinine and uric acid amd high blood pressure of around 170/90, then I went to kidney specialist and he prescribed febuxostat 40 mg for 3 months, and relmisart 40 mg twice a day 1-0-1, with amlodipine 5 mg once a day 1-0-0, now its been 6 months I am eating these, and stopped fexanto, and now due to lighten, from past 1 week, I am checking my blood pressure at home, so it is normal 115-80, hence now, can I reduce my dosage of telmisartan 40 mg to once a day, 1-0-0 and increase amlodipine dosage to twice a day, 1-0-1.because I have heard that telmisartan declines kidney function and is not good for kidneys for long term use. Ps: I cannot visit kidney specialist doc as he is stuck out of india because of cavity 19 lockdown. Please help me out with a detailed reply.

I have seen the details given. I do not know from where you have gathered the information about Telmisartnan causing "kidney damage". In fact these group of drugs (ARBs) and ACE- Is are considered safest of blood pressure reducing drugs. Since your BP is well controlled, there is no reason to reduce the dosage. Telmisartan is shown to reduce proteinuria and help improve Glomerular Filtration Rate (GFR), which an indication of kidney function. Thanks.

There is no time restriction on usage of febuxostat. It has to be taken till your uric acid level drops and then as maintainence needs to be continued at lower dose.

PYrazinamide induced hyperuricemia.
You need to start taking febuxostat.

I am diabetic and hypertensive for last 20 years. Also has high triglycerides. All are in moderate control and in medication regularly. My challenge is gout as my uric acid levels are always between 6 and 10 and keep getting gouty attack once a month on toe, ankle or knee. I will become completely immobile for 3 to 5 days. My medication: vobit m 0.2 mg - 1-0-0 onglyza 5 mg - 1-0-0 ace revelol 25/2.5 - 0-0-1 zyloric 100 mg - 1-0-1 rozucor asp 10 mg - 0-0-1 I have hb1ac at around 7.5 bp is normal after regular medication lipid profile is also normal. Whenever I consume even 2 legs of whisky (even once a month also) I get gout. My protein diet is limited. But we eat lot of udid dal breakfast like idli and dosa. My kidney and liver function is normal. I do master health checkup including TMT every year. Moderate physical activities. Vegetarian diet. Let go fish-eating also. Please suggest what else I can do to control my gout.

Dear Doctor - I have kidney stone of 2.8 mm in midpole of right kidney. I had total 5 of them since last year but all except this one have cleared with medication. I was working out in gym and taking protein powder (70 grams) + egg whites+BcAA (branch chain amino acids), but I stopped taking them 1 year ago after kidney stone. My suspicion is that these were Uric Acid stones, however I could not see any while they moved out of my body. I am currently NOT taking any medicine but drinking water around 3 Litre per day. I took clonagem for high heart rate for couple of months. I got a KFT done today and Uric Acid is 8.1 (i do not eat non-veg, junk food, too much protein like pulses) and go to gym everyday doing moderately good workout. Could this uric acid be due to clonagem OR it is the stone which is causing this high. I took febutaz 40 for 2 months and that time uric acid was fine. Kindly help. Too many complexities and unable to find what might be causing the issue.

low salt diet continue febuxostat uric acid should be within normal limits
metabolic evaluation should be done
