Yes you can switch to losartan and cilnidipine.
Losartan has benefits in reducing uric acid also. It will control bp and prevent remodeling of heart too. Adding cildnipine will further help to control bp by dilating blood vessels and reducing work load on heart.
The mainstay of treatment for lvh with diastolic dysfunction is ace/arbs, diuretics, ccbs and beta blockers!
beta blockers may later be added if needed!

Hello doctor, I am 23 years old male and around 8 months ago, in my blood test I found high levels of creatinine and uric acid amd high blood pressure of around 170/90, then I went to kidney specialist and he prescribed febuxostat 40 mg for 3 months, and relmisart 40 mg twice a day 1-0-1, with amlodipine 5 mg once a day 1-0-0, now its been 6 months I am eating these, and stopped fexanto, and now due to lighten, from past 1 week, I am checking my blood pressure at home, so it is normal 115-80, hence now, can I reduce my dosage of telmisartan 40 mg to once a day, 1-0-0 and increase amlodipine dosage to twice a day, 1-0-1.because I have heard that telmisartan declines kidney function and is not good for kidneys for long term use. Ps: I cannot visit kidney specialist doc as he is stuck out of india because of cavity 19 lockdown. Please help me out with a detailed reply.

Hello, Thanks for the query.
I have seen the details given. I do not know from where you have gathered the information about Telmisartnan causing "kidney damage". In fact these group of drugs (ARBs) and ACE- Is are considered safest of blood pressure reducing drugs. Since your BP is well controlled, there is no reason to reduce the dosage. Telmisartan is shown to reduce proteinuria and help improve Glomerular Filtration Rate (GFR), which an indication of kidney function. Thanks.

There is no time restriction on usage of febuxostat. It has to be taken till your uric acid level drops and then as maintainence needs to be continued at lower dose.

hello allen..
as you said you are having gouty arthritis,,i would advice you to start allopurinol or febuxostat along with colchicine.....apply ice over the toe...consult privately for detailed prescription..thanks

PYrazinamide induced hyperuricemia.
You need to start taking febuxostat.
