Headache and fever persistent for 3 days should be evaluated first. Get your basic blood reports done like Haemogram done and consult with your local doctor.

Two week is quite a long duration to wait. As per the symptoms you have mentioned here, it seems like your tooth has an acute infection. I would rather suggest you get the RCT initiated as early as possible.
Your pain will reduce significantly after the first session on RCT since it will reduce the inflammation that is build up inside the tooth.

Don't give such medicine it has diclofenac really stressful on liver. Azithromycine require monitoring that's not very safe. To add antibiotics I will suggest you to give proper history about d condition. For d time being you can give 1) tab dolo 325 mg sos 2) plenty of fluids 3) rest.
