You have to adjust insulin doses after consulting diabetologist and switch over to other oral drugs since it is not working
1. Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc)
2. Reduce weight if overweight
3control diet

Anal fissure is due to injury of the mucosal lining of anal canal by hard stool caused due to constipation. Take fiber rich diet, drink plenty of water, avoid spicy food and follow the treatment of your treating doctor. Anal fissure can be treated by non surgical approach by graded kshar karma which has better efficacy and is less painful.

You should continue. Flares in the initial stage is common. Also start an OC Pill like Diane 35 or Krimson 35 which will help with your PCOS as well as acne. That way, Isotretinoin can be gradually sotpped after a few months and you can have a holistic treatment.

It could be due to gastric INFECTION
It requires 4-5 days to recover

Drink plenty of water
avoid spicy fatty oily food
take more curd butter milk and pomegranate in diet
take small frequent meal
Take antibiotics, prebiotics and antacids.

Take isphagul powder 2-3 tsp with a large glass of water every day at night , continue for at least 3 months , keeping stools soft will prevent future episodes of bleeding and on long run will improve your symptoms
