Yes it is time that you should be concerned about what is the actual problem that is causing all these symptoms for your mom. These symptoms can be either due to heart or kidney, but for that we need to get her investigated. You should get the following tests done - RFT, Lipid Profle, LFT, ECG and a good Echo and the consult a cardiologist near you or you can take her to cardiologist and then get a proper evaluation done.

Apart from medicines you need to follow a strict low salt diet and regular exercise. Avoid eating packed foods as they have increased sodium content. Inspite of this if blood pressure is still high then you need dose adjustments.
