Good evening sir. I am ramesh age 40. I am a pharmacist and rural doctor. I am suffering from dysthymia, anxiety neurosis, short temper and angry (soft mania) last 18 years. I consult psychiatrist and tried ssris, snris.due to side effects every 3 months I discontinued the medicines. I have also overactive bladder, l5-s1 slipped disc, gynecomastia,abnormal triglycerides, cholesterol, my weight is 83 kgs, stomach is 38 inches. But no sugar, b.p. No heart problems, thyroid.4 months back psychiatrist gave me duloxetine20, oleanz 2.5 mg,divalproex500 mg. Due to sexual side effect I discontinued. I try paroxetine, mirtazapine,desvenlafaxine, venlafaxine many more. According lancet journal2020 I know very effective and tolerable antidepressants simuntenuosuly agomelatine, amitriptyline,escitalopram, mirtazapine,paroxetine, venlafaxine.my favorite drug is escitalopram. But I taking escitalopram I got stomach upset and loose stool .is loose stool is dangerous. How many weeks overcome this problem sir. Is olanzapine or quetiapine safe for my body. I fear lithium horrible side effects. Jam live in village .here no psychiatrists. I lose my business. I am very very lazy, severe procrastinator, no hope, no energy insomnia, nightmares,etc, etc. For overactive bladder can I take tab mirabegron with antidepressants, tab solifenacin with olanzapine, quetiapine.psychiatrist gave me tab modafinil for my lazy. Sir I request you to which antidepressant, and which antipsychotic ,mood stabilizer is effective for my problems. Last 2 months I have no take ssris, snris.i am a pharmacist no problem to you. please sir give treatment guideline for me.

Hi, by the list of medications I can understand that you have tried a lot of medicines. Still your problem is not resolved. Since you are near to bangalore the best way I could suggest is visit nimhans bangalore and consult a psychiatrist and a therapist for your current problem you might need more of therapy and a different kind of antipsychotic which works for dysthymia. The medicines which you have mentioned are antidepressants and does not necessarily work in dysthymia.

Duloxetine is an antidepressant and is given for pain also .It has been prescribed for periods like 6 weeks.
If there is a problem in l4. l5 only this medication alone will not help.

MRI of L-S Spine will help in diagnosing and treating it well. A short course of Non steroidal anti inflammatory drugs like Naproxen may be taken to reduce pain. Antineuralgic drugs like duloxetine may be required as well.

Of course you must be knowing about kegels exercises. Sometimes duloxetine 20 mg twice daily also helps but if nothing works then we put propene tapes around urethra surgically to support it so that urine leakage stops.

See a good neurologist. First thing would be to rule out other causes for the pain especially the tongue pain. Second, There are other medications for a small fiber neuropathy which is what your father seems to have. Some of them e.g. Duloxetine may help your father.

Hi, I m a 34 years old female. Who experienced mild pain in Head and tingling in fingertip of Right Hand only and numbness on right side of face and lower lips right side only from last 1 month. One of my friend advise me to consult with Neurologist, and after consultation with doctor, he advised me to take MRI and EEG test of Brain. After seeing MRI & EEG reports doctor said that it is :- A well defined ring enhancing lesion is seen in the left thalamic region is to hypointense core on T2 & FLAIR with T2 Hyperintense rim. The lesion shows peripheral Restriction on diffusion weighted images. The lesion measures approx 15x15x14 mm (APxTRx CC). There is significant perifocal edema extending into left basal ganglia, left cerebral peduncle, mid brain & pons. Rest of the brain reveals normal signals and gray white matter differentiation. All other part of brain functioning normal. Impression: Mr. findings reveal a rim enhancing focal lesion in left thalamic region with significant perifocal edema. appearance may represent inflammatory granuloma? Tubercular? neurocysticercosis. Advise clinical & lab correlation. My Doctor has advised me to get admitted into hospital for 1 week to diagnosis or treatment (Full body checkup). They give Antibiotics and other medicines and specially medications used to treat TB (Tuberculosis) include: Oxetol 300 mg, R-cinex 600 mg, Benadon 40 mg (1/2 Tablet), Caltok, Pyrazinamide (1500 mg), Combutol 800, Medrol 4 mg, Levipil 500 mg, Lumia 60K, Gabapin, duloxetine 20 mg, ARQ-10, Nuhenz, Numethoxa. As I said I was admitted for a week and taking all the Body test (Like sugar, x-ray, thyroid, blood, BP and others) which are prescribed by my doctor are Normal. Now after discharge from hospital Im taking all these above medicines on daily basis from last 2 weeks. Most of these medicines are use for treatment Tuberculosis. Its like 3 weeks im taking these medicines on regular basis and consulting with my doctor once in a week for the same. As I like to know is im getting correct treatment? Also is there any side effects of above medicines, Above drugs must be taken for up to 1-2 years? And last is Can this TB infection on brain be completely cured? Please suggest me what I have to do and if there is any powerful medicines or better treatment. Thanks! Many Thanks! simple.

Yes TB can be cured If you take treatment properly. Some medicine are for TB some for preventing side effects to TB medicine and some for preventing neurological complications of TB itself.
The medicines will continue for 12 months to 18 months.
