I am 56 years old male undergone Angioplasty five years back due to severe attack and was put two stents. I was alright for first 15 days and thetesfter, I have been suffering from pain at my chest continuously. at times, pain is like pinching, sometimes stabbing like and sometimes hammering like. I am not able express the exact nature of pain and particular point. I have undergone many tests viz, ECG, Echo, Xray, TMT etc. Several times with famous Csrfiologists, reports are negative. But pain still persists 24/7 through the yesr. Doctors have prescribed many medicines to get rid off the pain like Zerodol, Dulane, Xykaa, Amunarite, but in vain. Further, I have been taking clopitorva 20 mg a tablet each every dsy snce Angioplasty as prescribed by Doctors. During April end this year, to clear the doubt, Doctor suggested to undergo Angiography test which I underwent in very famous KLE Hospital, Belgaum (I reside in the same city in Karnataka) and report is normal. L. Further, I approached Apollo Hospital at Bangalore last month and they have suggested various blood tests like Vitamin. D, calcium etc. But all the tests are within normal. When all the test reports are normal, why the pan remains that too intolerable. I am living in fear day in and day out. I am totally frustrated due to unexplsinale, intolerable pain. I walk around half an hour daily. I do not get good sleep, due to which I am developing weakness. I am vegetarian and take good diet. My BP is normal and am not diabetic. Please suggest proper solution.

Hi, by the list of medications I can understand that you have tried a lot of medicines. Still your problem is not resolved. Since you are near to bangalore the best way I could suggest is visit nimhans bangalore and consult a psychiatrist and a therapist for your current problem you might need more of therapy and a different kind of antipsychotic which works for dysthymia. The medicines which you have mentioned are antidepressants and does not necessarily work in dysthymia.

Duloxetine is an antidepressant and is given for pain also .It has been prescribed for periods like 6 weeks.
If there is a problem in l4. l5 only this medication alone will not help.

MRI of L-S Spine will help in diagnosing and treating it well. A short course of Non steroidal anti inflammatory drugs like Naproxen may be taken to reduce pain. Antineuralgic drugs like duloxetine may be required as well.

Of course you must be knowing about kegels exercises. Sometimes duloxetine 20 mg twice daily also helps but if nothing works then we put propene tapes around urethra surgically to support it so that urine leakage stops.

See a good neurologist. First thing would be to rule out other causes for the pain especially the tongue pain. Second, There are other medications for a small fiber neuropathy which is what your father seems to have. Some of them e.g. Duloxetine may help your father.

Duloxetine is used to treat depression and anxiety. In addition, duloxetine is used to help relieve nerve pain (peripheral neuropathy) in people with diabetes or ongoing pain due to medical conditions such as arthritis, chronic back pain, or fibromyalgia (a condition that causes widespread pain).
Duloxetine may improve your mood, sleep, appetite, and energy level, and decrease nervousness. It can also decrease pain due to certain medical conditions.
