Hi Doctor, good morning. Myself a 25 year old male, am feeling from last few months that may be I have some problem with my heart or I hv developed hypertension. Few symptoms I am experiencing are:- stiff neck muscles from 3 months, pain in heart area from last month when I am getting tensed, trouble in sleeping, confused, slurred speech, easily getting irritated, dry mouth, feeling hot red ears in ac, cough in mouth all the time specially in the morning after I wake up. I hv stiff muscles in lumbar region also, for which I hv to twist on bend and pop them to get relieved. I hv orthopaedic problem also. Before 1 year, my Vit d was 25 iu and Uric acid 7.0. Are these happening due to anxiety or some heart problem? I have attached my recent full body blood test report. This is my full body blood test doctor. eosinophil-11.6 (0-6.0), eosinophil - ABSOLUTE COUNT 0.59 X 10³ / µL (0-0.5), HOMOCYSTEINE 38.9 µmol/L (<30), HIGH SENSITIVITY C-REACTIVE PROTEIN (HS-CRP) IMMUNOTURBIDIMETRY-6.1 mg/L (<=3.0), folate-3.7 ng/ml (> 5.38 ng/ml), bilirubin direct -0.31 mg/dl (<0.3), albumin serum - 4.9 gm/dl (3.2-4.8), 25-OH VITAMIN D (TOTAL) C.L.I.A- 17.75 ng/ml (DEFICIENCY:<20 ng/ml). VITAMIN B-12 C.L.I.A 304 pg/ml (211 - 911 pg/ml) Please tell me whether these are normal. I will be very grateful to you. My mother has hypertension from 4 years. Thank you.

According to me your heart is in good condition. Can you relax your stiff muscles. You start with ASHWAGANDHARISTHA 10 ml + DASMOOLARISTHA 10 ml + water 20 ml after lunch and dinner. Also do oil massage followed by warm water bath.

My mom is not well from last 1 month or so. She is under medication and yesterday as per doctor advice we have do a bone marrow test. Need your help in understanding the report and the treatment if any to cure her. Bone marrow report details are peripheral blood smears: total leukocytes count is within normal limits. Differential leukocytes count shows neutrophilia predominance. Platelet count is markedly diminished. Erythrocytes are normoochromin normocytic. Anisocytosis present. Comments: bone marrow smears reveal megaloblastic and dysplastic changes, and increased storage iron. Please exclude systemic disease, and deficiency of vitamin b12 and folate, and follow up. In absence of above condition, the possibility of myelodysplastic syndrome should be considered, if cytopenia persists. Report on bone marrow: clinical history: bicytopenia with low grade fever. Cellularity: cellular marrow bits and cell trails seen. erythropoietin: a significant number of erythroblasts show megaloblastic and dysplastic changes. Myelopoiesis: myeloid maturation is mildly left shifted. Giant myeloid precursors and a few dysplastic myeloid cells seen. Myeloid-erythroid ratio: 2: 1 megakaryocytes: megakaryocytes are adequate in number. A fair number of meagkaryocytes are hyperlobulated. Plasma cells, lymphoid cells & blast cells: within normal limits haemoparasite: none seen perl's stain: stainable storage iron is increased. No ring sideroblast seen. Need your guidance and opinion.

Firstly, it has been recommended to check for vitamin b12 defficiency and other diseases if she has already. For this vitamin b12 level should be tested in blood. If these two points are okay, then she will be diagnosed as having myelodysplastic syndrome. Mds are conditions that occur when the blood forming cells in the bone marrow are abnormal. Surgery is the gold standard for gall stone disease. Kindly get it done before it gets complicated. Stomach pull seems to be due to gall stones. Generalised weakness may be due to low food intake or feeling of unwell ness due to known disease (gall stone in your case) in the body.

If your heamoglobin is on a lower side than only we call it anaemia otherwise it is just a case of lower VitB12 and folate levels

Diagnosed as Moderate Obstructive Sleep Apnoea. Hello, From last 11 years I have been suffering with Morning drowsiness, Fatigue & day time sleepiness for 15 days per month where I work more (mental and physically). From last 1 year, I thought to fight against it as I am unable to do my daily routines with this problem and hence I consulted NIMHANS, Bangalore and they came to conclusion that Depression and started T. Venlafaxine 225mg. I undergone sleep study (polysomnography) and finally it came as Moderate Obstructive Sleep Apnoea. Respiratory Distress Index is 28/hr. The doctor said that brain is awakening many times though you are not waking at night due to which I am unable to relax fully and hence the fatigue and day time sleepiness. Does this Sleep apnoea can show as more fatigability, drowsy, morning sleepy when I do more work? Whats the relation between more work and next day drowsy if I have daily sleep apnea, does the body need more rest during more work and hence the problem is being faced at heavy mental work? why this problem cant be neurasthenia as that also occurs at when more mental & physical work is done? The doctors saying that I need to go for CPAP titration and then to CPAP machine. Over internet I read that oral based appliance is also being used for Sleep apnea, which one is best, user friendly and cost effective? Can I be fully normal after using CPAP machine as I being diagnosed as psychiatric until now?

I will suggest you to do only one blood test called as - serum folate levels with erythrocyte folate level and revert back with reports
