You can apply xylocaine nf ointment three times a day and take sitz bath for 10-15 min three times a day. Inadequate dosage of ointment is the main reason for persistence of fissure.

This may be due to internal hemorrhoids or bleeding piles for this you need to consult a doctor and go for per rectal examination, and if piles are present you can go for kshara Sutra treatment for piles which will cure it permanently.

It is a sentinel pile most likely. Develops after anal fissures or due to prolong constipation. Do sitz bath, avoid spicy meal. High fibre diet and avoid straining during motions.
