Usually fissure in ano occurs posteriorly but if it occurs anteriorly then other causes like TB, crohn's disease etc has to be thought about. Or rarely maybe a fissure which has come anteriorly without any of these issues. So evaluation for TB also has to be performed .or take usual treatment for fissure like dilzem ointment three to four times per day.

Please do not use the steroid ointment. It will make your problem worse. You need lifestyle modification by taking lots of fluid and change your diet to prevent constipation. Also check your thyroid level which maybe cause for constipation. Try to take laxatives if you feel stool is hard, and try lignocaine and diltiazem ointment. Consult for further queries. Thank you.

Have you tried diltiazem containing ointments and warm sitz bath. If no den start applying this ointment and do sitz bath. Use more roughage in you r diet and plenty of water. If this measure doesn't makes any change and symptom persist even after 15 days. Get a small surgical procedure (closed lateral sphincterotomy) done that ll b a permanent relieve.

You might have an anal fissure. If you have bleeding in stool while defecating it is an anal fissure. If you have Frank bleeding without pain after defecation it is hemorrhoid. So make your stool soft by having lot of fruit and fibre in your diet. Drink lot of water. You can avoid constipation by following Gud fibre diet. If you have anal fissure use diltiazem ointment that relaxes the muscle.
