Seems like you have a fissure-in-ano which requires corrective treatment. Please use diltiazem cream and apply it over the anus at least 15-20 min before you pass motions. If the pain is persistent and doesn't subside, you need to see an ano-rectal surgeon who will be able to sort you out.

Although there is no specific treatment for the same but I will advise you certain steps u should follow. May be these can help, drinking water before, during and after meals. Eating small bites of food. Chewing thoroughly before swallowing. Avoid talking while eating. Eat slowly. Walk, stand and sit upright for at least 30 minutes after eating. You can also take a homoeo medicine along with nux vomica 30 ( dr. Reckweg ) 1 drop in 1 spoon fresh water twice daily. Report me after 2 monthss

U seem to have defeciency of folicacid in your blood and so take tablet folvite 5mg once a day for six months and vitamin d3 60000iu once a week for six months and can reduce symptomatic treatment tablet after a month.
