Diltigesic is same as diltiazem. Apply it outside after taking sitz bath i.e. Hot water fomentation.

You should take some stool softening medicine like Haritaki Churna/ Sushruta Churna in night with warm water so that bowel movement is comfortable. Passing hard stools may cause anal fissure that is small cut in anal skin, which leads to burning or itching and blood in stools. To avoid constipation drink plenty of water 3-4liters / day, take high fiber diet and do physical activity. To treat constipation, you have to take laxatives till fissure heals.
