If you take 30 tablets of digoxin mixed in a solution you will get irregular heart beat and and heart beats will vary and you can lose consciousness

I am 51-year-old patient with a 10-year history of hiv. I am on lopinavir+ritonavir +lamivudine+zidovudine tablets along with ayurvedic medicine. I am also on cardiac medicine for enlargement of heart, which are lesilactone, digoxin and ecosprin. Presently my viral load is at undetected level and cd4 is 266 I am having chronic back pain in lumbar region and coccyx, which typically subsides with use elemental iron. I request you to kindly help me to diagnose this and suggest me treatment ?

I am 65 year old female. I was replaced with prosthetic mitral valve in 1990. I am on warfarin/a intron since then and monitoring INR monthly for adjustment of warfarin/ acitrom. Since last about 4 years I on Acitrom 2 mg daily. I was operated for subdural clot in brain due to fall while walking and was operated for its removal in 2000. I am taking metolar 50mg daily on that account. I am also taking digoxin 1/2 daily. I am now advised to go for cataract operation by eye surgeon in right eye. Is there any risk for the cataract removal surgery. What precaution are required during surgery. Any special advice for cataract surgery.

todays cataract surgery is predominantly blood-less so there is hardly any chance of bleeding as surgery is usually performed using anaesthetic drops and without any injection. only thing that you need to take care is have a proper physician checkup before you plan surgery and have a anaesthetic standby rest no other special advice

Colchicine, pantop and digoxin can often be stopped and used intermittently. Others must not be stopped without medical supervision. Substitutes of losar are often preferred in this problem.

Hi, I am 25 years male. I'm suffering from severe mitral valve stenosis (orifice area - 1 cm. From last month whenever I push my nipple tip, I feel like a pinching pain only in nipple tip. You need to see a cardiologist. you have not elaborated your problems clearly. Might b gynecomastia related to digoxin if you r taking this drug. Severe mitral stenosis can b treated effectively depending on d nature of valve affection either by open surgery or closed heart procedure. Consult as early as possible.

They had heart attack 2 year before and we checked his recently taken echography test report then we found ejection fraction is 35-40%. Please advice me how can increase EF % for normal heart pumping.

There is no real way to increase the ejection fraction which has gone lower due to the heart muscle damage suffered in heart attack. You have to live healthy lifestyle and relax, Regular check up is needed.

Pep is required for all persons invo5 in providing medical and surgical services 
No environmental transmission reported but pep is pep is required when they are exposed to -

 potentially infectious body fluid e. G. Blood, semen, vaginal secretions, synovial, pleural, pericardial, peritoneal fluid, amniotic fluid. Any bodu fluid contaminated with" visible blood" considered" at risk"
 exposure to body fluid considered" not at risk" e. G. Tears, sweat, urine, faeces, saliva, sputum and vomitus

Management of exp6 site
- do not panic 
- wash wound and surrounding with soap and water 
- rinse well
- do not scrub
- for not use antiseptic or skin washes
- splash of blood 
- irrigate eyes with water or saine
- if using contact lens leave them in place while irrigating. Remove the contact lens and clean them after eyes is cleaned 
- mouth: spit fluid immediately and rinse mouth thoroughly water/saline repeatedly 

Pep is required within 72 hours of exposure 
 conslt nearest art center for selection of pep regime e. G. 

1. Basic regime of 2 drug zidovudine 300 mg bd 

2 drug regime is recommended if third drug is required then use nelfinavir
 arv prophylaxix - short term arv drugs is used to reduce hiv transmission from mother to infant
- all hivexposed infants should receive cotrimoxazole at 4-6 weeks of age
 nevirapine prophylaxix 
- mother- nov 200 mg at onset of labour 
- baby- nvp syrup 2 mg / lg of body weight within 72 hours of delivery
