Yes on finding any evidence of Ethambutol eye toxicity, Ethambutol needs to be stopped immediately and not to be given again. An eye specialist should document the same. If required it should be replaced by another Antitubercular drug preferably with known sensitivity.

TB medication should not be interrupted as in your case. Rise in liver enzymes is acceptable till 3-4 times rise.

The doses of TB medications are according to body weight with a maximum recommended dose. Some times doses need to be modified as per kidney function test. I think you need to reconsult your doctor for doses as per body weight as per guideline.
