What next in a patient treated with 4gms ceftriaxone Tazobactum for14 days for typhoid with fever still 99F.Treated for malaria before this with quinine sulphate and doxy for 10days with nimesulide ,paracetamol,diclofenac off and on for fever.Now Widals still+ 1:80 and lft showing Sgot&Sgpt 777 and 253 and bilirubin1.8which in the start was normal .Widal is down from 1:160

Please stop the antibiotics, take plenty of fluids, increase your carbohydrate intake and sugar I take if you are not diabetic , I also recommend u/s upper abdomen

Hi, my wife got fever, so when consulted to doctor and had urine test, found she had urine infection so doctor suggested some medicines (UTIKIT - probiotic and probiotic capsules and Cefakind 250). Doctor also advised to get urine test done for me to see if there is any infection, so I had too done the urine test but my doctor is unavailable and out of station for 2 weeks. So I am posting the report details here, so please check and suggest me if I too had any infection and if so please suggest what to do. Report Result: Specimen: Urine Smear Report (Gram Strain): 3-5 pus cells, 1-2 epithelial cells and Gram negative Bacilli were seen. Isolate - 1: klebsiella oxytoca COLONY COUNT: >1, 00, 000/ml ANTIBIOTIC SENSITIVITY Highly Sensitive to: Amikacin, Aztreonam, Cefepime, Cefotoxime, Ceftazidime, Ceftriaxone, Cefuroxime, Gentamicin, Imipenem, Levofloxacin, Netillin, piperacillin/Tazobactam. Moderately Sensitive to: Cefoperazone Resistant to: Amoxyclav, Ampicillin/Sulbactam cefaclor, Cefixime cefpodoxime, ciprofloxacin co trimoxazole nalidixic acid, nitrofurantoin, norfloxacin, ofloxacin, tetracycline * End of Report ** Please examine above report and advise me if I too had any infection and if so what medications? Going by the reports, you need to treat both simultaneously and avoid coitus till future samples from both of you show no growth on culture. Treatment from your physician with a drug from the most sensitive to antibiotic group for sufficient duration is mandatory.

Hi, doctor my father age 45 he suffering from kidney problem his creatinine was 8.86.doctor advised piperacillin and tazobactam injection ip 4.5 g .so please sagest me they injection is controlled kidney problems and control creatinine presents or not.

The dose of piperacillin tazobactum needs adjustment, the drug is an antibiotic for treating infection, lowering creatinine and clearing nitrogenous waste products in ckd is donee only through dialysis or otherwise, best to consult nephrologist near you.
