Sir. My father had a heart attack on 30 Apr 17. He has been taking vildagliptin 50 + metformin 500 (OD) for the last 5 years for diabetes. He was NOT taking atorvastatin and aspirin. He has been smoking for 30 years. Angiogram shows a 90% block RCA. Angioplasty done with 2 stents and discharged on very next day. And advised to take aspirin 150 OD Clopidogrel 75 OD metoprolol 25 OD Atrovas 40 OD Metformin SR 1000 BD pantoprazole 40 OD. He stopped smoking then. AND after taking these medicines for a week his sugar level came down to 55 and 60 twice. So metformin reduced to 500 BD. Then also he is facing hypoglycemic conditions. Is vildagliptin safe to take now or continue with metformin 500 BD? And these medicines are for life long?

Hello doctor I want to ask you for my father treatment, he is 82 years old he is suffering from kidney problems, heart problems ,diabetes, high blood pressure from past 10 - 12 years now he is under treatment and continuing tablets (1 yr) for all these problems from gh chennai. Now the problem is urea and creatinine level is getting high urea is 61 and serum creatinine 2.34. How can I reduce it. He is consuming salt per day 3gm and water 1.5 litre. 20 days back we checked creatinine 1.87 urea 60 now creatinine 2.34 and urea 61 why it's getting high and low electrolyte are in control how can I reduce both. These tablets now using for 1 yr. Metformin 500 mg (m-n, aspirin150 mg (a, clopidogrel 75 mg (a), ranitidine 150 mg (m-n), metoprolol er 25 mg (m-n), atorvastatin 10 mg (n) 2 tablet, acitrom 2 mg (6 pm) .pls give me a good solution.

Hi. Increase in creatinine happens due to dysfunction of the kidney. Looking at your history it looks like your father is having chronic kidney disease. Some contribution to everything is partly his aging process as well. However he will need evaluation for the cause of worsening from 1.8 to 2.3. Also his diet seems to be reasonable. Pls meet a nearby nephrologist immediately.

I weight around 115+ and I got two stunts fitted in one vessel in Feb 2017 I have been taking Drug Rosuvastatin earlier 20 mg now 10 mg along with ecosprin any clopidogrel 75 mg each to reduce cholesterol and thinning of blood. I do a lot of cycling to reduce weight but due to cycling develop osteoarthritis type pain in bones which appears to be a side effect of rosuvastatin also but if I leave cycling I quickly gain weight can't do other exercises because of knee pain due to cartilage damage what should I do?

I am diabetic since 18 years and hypertensive since 12 years using Glimulin MF Forte in the morning janumet 50 / 1000 in the afternoon glimulin MF in the night. Conquer a.m. In morning gabapin NT 400 twice daily ecosprin AV 75 in the afternoon Telma 40 in evening. Recently after my health check up the doctor replaced Telma 40 with olmezest H 20 mg and rosuvas F 20 mg instead of ecosprin AV 75 mg and clopitab 75 instead of ecosprin 75 mg The health checkup revealed that my triglycerides level 212 and HB a1c is 6.93. Vldl 42. Coronary CT was advice because of my positive family history towards heart attacks The coronary CT angiogram reveal that the D1D2 arteries are having 60to70% blockage the calcium core is 281.2 LAD has a mild luminal narrowing The doctor advice me coronary angiogram. Is the coronary angiogram necessary, can I wait for 6 months of time or is it to be done immediately as I am asymptomatic. And is the stent necessary? Considering your family history, comorbid diseases and reports, you was correctly advised by your doctor. If asymtomatic then no urgency for angiogram. If at all angioplasty is advised after angiogram, better to go for CABG as in diabetic the coronary disease is diffuse. And angioplasty may fail.
