Asked for male, 60 years old from Pune

This is a followup question for Dr. Anunai shrivastav who answered my question recently. He suggested to stop ramipril and take olmesartan 40 mg. Thanks for that. I will be obliged if he can tell me, if I can take telmisartan 40 mg which is consumed by my wife and is readily available with us. I can try that for a week if doctor has no reservations. Advice if amlodipine also can be continued with telmisartan. For olmesartan 40 mg ,i do not have prescription. I paste below my earlier question for reference as below:- I am diabetic on insugen 30/70 refill injection, glycomet gp2, ppg 0.3 mg and other drugs. I am also hypertensive on ramipril 5 mg and amlodipine 5 mg. My query is related to hypertension and not on diabetic problem. I am taking the above medicines for last 5 years on hypertension but my hypertension is more than 160/75. It shoots to 175 frequently. I began treadmill jogging exercise for 1 hour every day and then hypertension reduces to about 150/72. If I do not jog for 1 day, hypertension goes back to 160/170. I am consuming ramipril 5 mg + amlodipine 5 mg everyday, once before lunch. The hypertension is not at all reduced with these drugs but only by jogging. I am 60 years old and jogging for 1 hour is very strenuous, leading to knee pain frequently. Suggest replacement drugs for hypertension.

best medicine to control blood pressure is exercise as you realize. As of now your blood pressure menu is having space to either increase dose or add one more drug. Here I will suggest to add one more drug chlorthalidone 12.5 mg.

Better way is to consult local doctor and have a check up. He will prescribe appropriate locally available medicines. Change of BP medicine has to be with check up and doctor's prescription.

It seems to be iron deficiency anemia so lupinfer is right. It would be right to get blood test for liver function blood proteins and vit b levels. Treatment would be according to cause. Check if you taking healthy diet. There should be no deficiency state.

Tablet cilacar contains cilnidipine which is a calcium channel blocker, helps reduce bp. Dilnip trio contains three medications which are combined together - cilnidipine, telmisartan and chlorthalidone. If your bp is 130/80 with dilnip trio then it means your body has adjusted to the medications. Your doctor would have started dilnip trio after much consideration, so it's advisable not to downgrade from dilnip trio to cilacar. Hello sir, My dad is 64 years old. He suffers from type 2 diabetes and hypertension. He takes olmezest am 40 for HTN. My question is - can he take OLMAX LN 40 as it contains another calcium channel blocker cilnidipine at 10 mg (5 mg cilnidipine is not available with olmesartan. As per my research on pubmed articles are in favor of cilnidipine as cilnidipine is an N-type and L-type CCB that also inhibits sympathomimetic activity in contrast to other DHPs and amlodipine therapy has adverse effects, most commonly pedal edema. So Cilnidipine being an N-type and L-type CCB is associated with lower incidence of pedal edema compared to the L-type channel blocked by amlodipine.

Clinically seen effect of Cilnidipine is different from Amlodipine in a lot of patients. Although, 10 mg of Cilnidipine is considered equivalent to 5 mg of Amlodipine, a shift from Amlodipine to Cilnidipine may lead to reduced efficacy atleast fir the first 1-2 weeks. You may change the medicines if unacceptable pedal edema is present.

Atenolol 200 is a quite high dose. Need to consult your cardiologist whether you really need it. Have a close monitoring of your heart rate.
