This is a very common situation in people suffering from Parkinsonism and you might be correct in assuming the symptoms of talking to herself are due to the medicines. Urinary disturbance can occur in long standing parkinson's disease. And coexistence of prostratomegaly can increase the problem. But for this it needs to be determined whether enlarged prostrate is contributing towards the problem or not. For this adequate evaluation is needed as there problems can be treated with medication.

I can understand your worry
with bypass and Parkinson's we need to see whether his brain is normal or not. So go for a MRI scan of Brain. Also, we need to see his memory and what are his thoughts, a history of how he has been since 2001. I think this would come up to diagnosis and then management of his condition.

Your father seems to be in quite advanced stage of the disease. In this stage neuro-rehabilitation is very much warranted. If he has any mood issues, those need to be addressed. Kindly consult a good neuro-rehabilitation physician. At i-nk we have a good neuro-rehabilitation department.
