paracetamol 250mg od & sos x5days do back (spine)/shoulder/knee exercises

This could be due to lumbar spondylosis with nerve root compression.
Control diabetes & rule outvit. D deficiency or any other metabolic disorder.

Swelling may be due to soo mny causes soo that has to be detected by observation only. So you will hv to visit in person to a physician fr this purpose.
