Hi, I have bilirubin 1.89 direct bilirubin 0.38 indirect 1.51 AST 91 ALT 90 GGTP 114 ALP 147 Creatinine 0.64 chloride 100. I drink alcohol almost daily is it risky? Can I recover from same after quitting alcohol any side effects occurs after quitting alcohol. Please give me better suggestion.

Immediately stop consuming alcohol. Some withdrawal effect may occur but they are very less than a liver failure. Get some psychological counselling done beforehand.

I am a 28 years old male. I noticed that I urinate more often than normal. I took a urine analysis test which indicated no presence of protein, bilirubin, urobilinogen, ketone, glucose, leukocytes and nitrates. Chloride, sodium and creatinine levels were normal within range. However, there were microscopic traces of blood in my urine indicated as ++ in my test results. Ph was neutral. I still urinate more often. Do you think there is an underlying issue? Should I be worried?

Dear Sir/madam I diagnosed with TB on 6 jun 2018. I had plural fluid in my right lung so my doctor removed it and started the treatment of TB with AKT 4. I fill very well after the treatment started but from 22 jun 2018 again I am not filling well and the cough is again started. Today I was go to my doctor and tell him my cough again started. He advised me to do cheat x ray and ESR test. Todays ESR result is 92 which is 80 on 16 jun 2018. In chest x-ray also some amount of plural fluid is increased. Hence he prescribed me to 1.Dextromethorphan HBR and amylmetacresol lozenges 2.Moxifloxacin Hcl 3.cetrizine dihcl 4. becosules 5.AKT 4 6.pyridoxine Hcl. I want ask you that can cough increase after the treatment started or it may be due another reason.

I have been suffering from coughing for 1 month. For first week I have taken moxikind 650 mg twice daily. Then, I have taken azithromycin 500 mg once daily with cough syrup codistar dx and cetrizine for 5 days. It made my cough more worsen. Then, I took medisedyl dx cough syrup. It made my cough more worsen and also I started vomit. Then, the sound of my cough changed with a high pitch sound. I always coughed mostly at night. Then, after 3 day I continued with moxikind twice daily for 3 days. Then, I took jufex forte syrup and fifatrol tablet and cetrizine for 3 days. Day by day, my cough was getting worse. Then, I took dextromethorphan hydrobromide syrup thrice a day for 2 days and cetrizine at night. Then my cough get worse and worse. And currently I was taking levolin plus syrup and cetrizine and cefpodoxime proxetil tablet. It did not provide me any relaxation. Whenever I cough, firstly I inhale deep breath with a very annoying and weird sound and then I start coughing with very dense saliva. I am 14 years old boy. I have no relief.

Hi doctor, I am 41 y/m/ teetotaller. No diabetes, no hypertension and not on any medication. Medical history: have a sensitive digestive system with recurrent acidity/gastritis/ulcer kind of symptoms due to my sales/it job. Mostly manage thru diet and during flare ups, use ppis/antacids. Endoscopy and abdomen ct done few years back, which came out normal. So doctor suggested it could be ibs. Current issue: for the past couple of weeks, had a mild heaviness and a mild intermittent pain in the pelvic area. Consulted an urologist who suspected chronic prostate/cpps due to my sedentary lifestyle & has suggested rft, usg (kub and scrotum), uroflow, urine and semen culture to rule out. The ultrasounds were normal, urine and semen culture normal cbc – normal & most of the blood work values are also within range, except serum chloride which is 110 meq/l whereas the reference range is 98-107 meq/l. Creatinine is 1.02 mg/dl, serum sodium 148 meq/l, potassium 4.4 meq/l, bicarbonate 27 meq/l, uric acid 5.3 mg/dl and urea 19 mg/dl. 1.Is the elevated serum chloride a cause of concern & does it warrant any additional investigations? 2.Does these chloride levels suggest any kidney disease? 3.Since my pelvic discomfort is very mild & all investigations are almost normal, should I take any medications for this issue or just manage it thru exercise/diet? 4.How to protect and safeguard kidney from any future renal diseases? Sincerely appreciate your advice on the above.

Mild elevation in chloride should not be a cause of concern and further investigations are not warranted. No, your chloride level doesn't suggest renal disease. You should try to manage your pelvic discomfort with lifestyle modification initially, if possible. Fever with headache for 1 day 10 days back followed by giddiness and fainting. Since 7 days palpitation and breathlessness. Tested for covid 19. antigen test.test was negative. Hrct chest done score is 12. Favipiravir started 5 days before. Spo2 97- 98% since 10 days ,vitals stable. Medicine given are as follows. Tab doxy 1 bd x 5 days tab vitc+d3+zinc 1bd tab montelukast levocetirizine 1od tab predmet 16 mg od x 5 days then next tab predmet 8 mg od x 5 days then next tab predmet 4 mg od x 5 days tab favipiravir 1800 mg bd first day followed by tab favipiravir 800 mg bd x 14 days. Betadine gargles tds daily syp. Macberry (dextromethorphan hydrobromide) 5 ml tds tab ultavit (multivitamin). 1 od cap pantop-d 1 od tab cefuroxime axetil 500 1 bd x 5 days. Patient is not consuming diet well. What we can do further. Any changes in medicine? Any thing we can add? Patient has breathlessness after coming from wash room and it last for 10-15 min.

diet is most important in covid as it weakens our immune system. If your patient is not diabetic then she should take sufficient ors to keep herself hydrated. What is the oxygen saturation of patient when she is breathless?
As of now no changes in medicine is required.
Advice patient to do breathing exercise using incetive spirometer to increase the functioning of lungs.

Respected Sir, I am suffering from cough since 2 to 3 years. I was under treatment of a local physician but when there is no more relief I went to CMC vellore and was under treatment of Dr. Shakti Kumar Bal, DNB, Asst.prof - Dept. Of Pulmonary Medicine, CMC Vellore. While staying at Vellore I had no problem, but at the returning when I was At Bankura, Westbengal Railway Station then again the cough started, so after return at my home I visited a local physician He gave me some antibiotics and a Cough Syrup Trump-D. But it not work. Whenever I come in contact with cold air the cough started. So I consult the CMC Vellore doctor and he advice me to take MDI Seroflo 125 mch 2 puffs twice daily via spacer MDI Asthalin 100 mcg 2 puffs as and when there is cough in between the doses of Seroflo 125 mcg for three weeks. I am started the Asthalin 100 mcg 2 puffs daily from 01-01-2017 along with when there is cough in between the doses of Seroflo 125 mcg. But still whenever I come in contact with cold air the cough started. One more thing I want to mention that I am working in DAV School, Koyla Nagar as a Lab attd. At Chemistry Lab. One while the Practical was conducting for Std. XII students I have Inhaled Sulphuric Acid and Then Ammonium Hydroxide. After Two or Three days Itching in my throat was started. Please help me to overcome this cough. Thanking you.

Regular cold sneezing episodes could could be due to dns (deviated nasal septum) ent opinion and examination required. Your sugar levels are quite well controlled and metformin is first line drug for type 2 diabetes. You should continue that. Take it after breakfast instead so gastric symptoms will be less. Check your hba1c level. Try to keep it near 6.5. It is more reliable test.
