My approach would be to review the diagnosis and ascertain the incomplete improvement on above treatment. Just adding tablets may not be the ultimate solution.

Get your serum sodium and potassium level tested. Consult your psychiatrist about possibility of initiating psychotherapy with psychologist. That would help your symptoms and might help to decrease medicine dosage to some extent eventually. Causing relief from the symptoms you have narrated.

There are multiple issues like ocd, depression and obesity. All these are interrelated. I would like you to add 'walking _ brisk walking'to your treatment regimen. In 2-3 months you will see a much better. Version of yourself. Regarding medications, I would like to modify few things like add on a beta blocker and ssri with better side effect profile. if bupropion works well for you then you might have to assess how well it has worked for you. This is the difference between response and remission. If you are completely not free from depressive symptoms one can usually go up higher on the dose of bupropion to 450 mg, however I strongly suggest that you consult your psychiatrist. There are usually no long term adverse effect of this medication documented in literature. I hope this helps.
