Yes 230 ppbs is high.
Get your hba1c done. You can either add metformin or gliptins to take care of you post prandial high sugars. Erectile dysfunction can be due to several causes like nervous, psychological, atherosclerotic related which needs to evaluated via counselling, doppler and medication. First get your sugar under control.

Mr. lybrate-user, hello, thanks ror the query.
The fasting glucose mentioned is quite high. Ideally fbg should be < 100 mg to closer to it. Pp should be 150 to 170 mg & hba1c% < 7% at your age. Therefore, it is clear that your diabetes is not in proper control. So there is a need to look at overall treatment which includes dietary pattern, extent of daily exercise, lipid profile and drug therapy. So I need to know your hba1c%, lipid profile, and above mentioned information. So if you come ba...more

Mr. lybrate-user, thanks for the query.
I have seen the details given. Since there is no information on fasting, pp glucose, hba1c%, lipid profile, dietary pattern & extent of daily exercise, there is no way to give any suggestion. Secondly your bmi (body mass index) is 28.65 kgs/sq meter (normal < 23 kgs/sq meter) which means you have an excess of over 15 kgs. This is a major hindrance in better glucose control. Plus I do not understand the reason for changing the medication. Please come ba...more

Hello, Thanks for the query.
Tablets with either XL or SR mean the drug is released slowly over period of 12 hours or longer. Other tablets have normal releasing compound where action may last for 6 to 8 hours or longer depending upon type of the compound and its pharmacokinetic characteristics. . Thanks

Hello, Thanks for the query.
In patients with diabetes, uncontrolled high glucose levels are the main or the only reason for causing damage to kidney and leading to long term problem of failure. The antidiabetic drugs used (oral) are quite safe. There are patients on oral drugs for over 50 years without any damage to kidney. But if glucose levels are not well controlled that is sure to damage kidney filtration membrane and lead to end stage kidney failure. Thanks.

Mr aashish, you have mentioned" average glucose level" of 140 - 145 mg, what exactly you mean by that? is this fasting or after food level? please give me current fasting and pp sugar levels and hba1c%, also the level of physical activity done by you daily.
