Now in ipf/fibrotic, nacfil is not of much use. Prefinax 1800mg in devided doses is prefered drug now. Medicine life long. Not curable. No remission if ipf.

Hiatus hernia has very good response with surgery and relief from symptoms is quick. Surgery can be done laparoscopically with small holes.
But get her assessed first if she is fit to undergo surgery as you say she has othet assocated problems.
Otherwise medication as is going and some lifestyle change is the only solutuon, eg head end up when lying down by 30 deg, not bending forward. Regds.

Get a complete cbc, rft, and lft done, to find out the pattern of liver enzyme derangement. Then get hep b and c viral markers. Post transplant patients are prone to many infections and other chronic ailments. please consult a nephrologist.
