1) Your Bp is not well controlled you need dose titration and probably other combinations for better control so please consult your treating cardiologist for the same. 2) Stopping or at least reducing smoking is very important in your case for better BP control and so please take medical help of psychiatrist to help yourself in quitting. 3) Also ensure that you exercise for 40-45 mins daily and eat a DASH diet (take a detailed diet consult for the same.)

You seem to be overweight for your height. You should focus on proper food habits and digestion. Avoid fried and spicy items, timely food, avoid unusual seasonal vegetables and fruits and other activities, regular light exercise like walking, symptom relieving tablets like unienzyme. If no relief have a general check up from a physician.

You need to make sure that you don't have hypothyroidism which can lead to these changes of depression and obesity which can lead to further changes. The doses of the drug can be adjusted but for that I will need to look in details with all your reports. You can consult me with detailed history and all your available reports for better care.

The reason might be due to many causes. Commonest here would be a stroke involving temporal lobe, leading to aphasic speech (irrelevant), which can be confirmed by a mri scan of the brain. Second reason can be due to dementia, which happens to shrinkage of the brain affecting recent memory and which is caused/ aggravated by the sleeping pill, alprazolam.
