Asked for male, 26 years old from Lucknow

In february this year I was diagnosed with mixed anxiety depressive disorder with ocd (thoughts repetitive) by a psychiatrist in lucknow. I had 3 sittings with the doctor in which he prescribed medicines-tab arpizol 2 mg, tab lam plus h, tab edopin 50 mg, tab parotina cr 25 mg, tab larina-50, tab nicerbium 30 mg. I followed this for many months but in my 4 th setting I couldn't meet my psychiatrist as I was doing my internship and all the medicines stopped except lam plus h. Currently my issue is that- I feel little heavy in head and I feel little withdrawal of medicines. So what should I do?

Asked for male, 22 years old from Delhi

Patient is on the highest dose of paroxetine and also takes bupropion xl 300, in connection with anxiety and dysphoria. He has mild ocd which has subsided by medication upto 80 percent. Now to treat these mild symptoms doctor put me on antipsychotic (for chain of thoughts. Only small dosage was told to be enough to me. But, whenever I try any antipsychotic, it makes me devoid of emotions and anxious. We tried olanzapine 2.5, arpizol 2-5 mg and lately risperidone 1 mg. But I started feeling again emotionless. So is it possible for an antipsychotic to have such an impact. If yes, which medication to be taken for remaining 20% of thoughts (ocd). (tried clonil too). Thanks a lot.

Hello Ankur, my suggestion would be to start Psychotherapy side by side to manage your intrusive thoughts. Cognitive behaviour therapy will help you to challenge and control your intrusive thoughts and you may feel under control.

Names of some medicines which is prescribed by my doctor that I took in these 10 years are - duzela, revocon, sulpitac, prolidine, luramax, adesam, braingis, melzap etc. I ate all these medicines for the long term presently, my doctor prescribed me the following medicines-1. Present medications are newer generation antipsychotics they are relatively safe but any medication may show side effect on long term use. Further usage of medication depends up on the condition and assessing the risk benefit ratio.

My son is 29 years old. Dr. diagnosed him as schizophrenia since last 8 years. Now a days he is refusing to take medicines. Antiphyscotic medicine was going on. We are giving him risperidone 1.5ml morning, 1.5 ml evening olanzapine 15mg evening and aripiprazole 10mg morning. We were giving these medicines without telling him. But alternative episodes are according after about every 15 days. I want to know such type of medicine which stop the symptoms and stop the future episodes. Is any medicine exist which prevent this disease and a person could live its normal life. Is ETC a better treatment for schizophrenia? Frequently ETC is required for this disease? kindly intimate.

I am suffering from OCD. My obsessions include thoughts and images of someone close dying, unwanted sexual thoughts about closed ones or Gods, and in order to get rid of these obsessions I perform some compulsions including restarting my laptops & mobiles again and again but it also doesn't help. I am on Fluoxetine 60 mg and Aripiprazole 10 mg daily. But I think Fludac is not helping me now at all. I was also on Fluvoxamine CR 100 mg before Fludac. It was effective for me but not that much, so my doc switched to Fludac 60 mg. Now my doctor has prescribed me Clomipramine 25 mg daily at night. It is helping me quite a lot. I have a query regarding my medications. I am trying to contact my doctor but he is not able to attend my call right now. My question is, can I take Fluvoxamine CR 100 mg along with Clomipramine daily as both the medications are effective for me?

Hello,
Taking fluvoxamine with chlomipramine can increase blood levels of chlomipramine and thus can increase side effects. Do not combine drug until prescribed. Drugs if combined need to be increased gradually with caution. For any more queries you can consult me.

My LFT is high such as GGT 451, SGPT 96, SGOT 85 with USG of whole abdomen impression showing hepatomegaly with diffuse fatty infiltration (enlarged liver). My Lipid Profile of TG 301, TC 276, LDL 176, VLDL 195, Non VLDL 204.8, HDL 55 showing. What I should do? May I take Sorbiline syrup and Essentiale - L for LFT and Pivasta 2 for Lipid Profile? My mother has been taking- (3 tablets) mirtazapine 15 mg and 1 tablet - aripiprazole (10 mg), since 2 months now, the combination has been very effective in treating her ocd and depression, but since a week she has been experiencing some side effects like- weight gain, tiredness, motor skills not functioning slow, unable to walk fast, she also experiences brain zaps, even though she's taking the medicine regularly. Are these side effects common? How to deal with these side effects, when the medicine has actually helped her overcome ocd and depression a lot. Is changing medicine a good option?

Yes it could be the side effects of medicine she is taking. For ocd and depression there are better medications options are available with least side effects. Please discuss with your treating psychiatrist about other medication options such as ssri for ocd and depression which has better results and less side effects. All the best.

I am taking aripiprazole, fluvoxamine maleate extended and clobazam for ocd for the past 2 days at night prescribed by doctor. After taking it the first day night, from next day morning I started feeling sleepy, tired and also have diarrhoea two times a day and chills. Is this sleepiness and tiredness because of the tablets? And also I slept late at night at 12: 00 on that day when I took the tablets. So does the tablets cause sleepiness and tiredness because I slept late? And it's been 3 days and I am feeling the same sleepiness and tiredness. What should I do?

Hello, clobazam is never taken for a long period. Sleepiness is due to clobazam mostly. Try tapering off clobazam. For initial 7 days take half the dose then for next 7 days one fourth of the dose then stop clobazam.

If pathologic hypersexuality occurs on aripiprazole or cariprazine, and isn't mitigated by ssris, and changing drug is absolutely not an option, nor another antipsychotic an option even as adjunct, what can be given to help it? Cannot change drug as only one with awakener response. Diag negative schizophrenia. Taking cariprazine 3 mg, valproate cr 1g, fluoxetine 40. Hypersexuality comes like waves and floods in chest similar to adrenergic overactivity resulting in uncontrollable impulses and compulsive behaviour. Doctor is at a loss how to help. Please suggest in writing, not over phone, if anything can help.

Can try hiking up ssris, fluoxetine upto 80 mg and if these are episodic behaviors, then .maintenance ect can be tried. But with caution. But ect I would not have recommended if another antipsychotic was open as an option. Consult a psychiatrist for nearby for discussing best options. This decision is better taken after detailed assessment of impulsive behavior and trying out some behavioral methods would be possible only if patient is available for assessment.
