Bipolar depression is very different to normal unipolar depression and need very specific treatment. The treatment you are taking if not helping you, you need to take a second opinion from another psychiatrist nearby or online. Since You are a heart patient, it is advisable for you to take medications that have least effect on the heart. Paroxetine is a SSRI, which is considered to very safe for heart patients. On the other hand Zapiz is Clonazepam which is a benzodiazepine which should be used on a short term basis. They should not be taken for more than 4 weeks as they may cause dependence.
