The other substitutes which you are talking about are called NOACS : novel oral anticoagulants! Like dabigatran , apixaban etc Which do not need any INR monitoring and are better than acitrom in terms of bleeding .. but sadly they are not indicated in your case / or any pt who has an metallic valve replacement or rheumatic disease.. so follow your doctors advice !

Acitrom 2mg tablet is an anticoagulant. It acts by preventing the formation of harmful blood clots. Although it does not dissolve the existing blood clots, it inhibits them from growing larger and causing serious problems.
You can change the dose only after a consultation

As per the latest guideline by American college of chest physician, INR should be between 2-3 for adequate effect. Needs escalation of acitrom dose to get a target INR.

Only applying cream won't help probably.

It depends on the day of periods, and so on. If you miss periods do pregnancy strip test and consult gyne immediately. All the best.
