Hello,
First of all you should take rest for 1-2 days so that your body can fight with underlying infection with its full immunity and start the below medication:-
Take gelesemium 200, twice daily .
Take Eupatrium perf 200, 5 drops thrice daily.Take Antifebrinum 3x, 2 tabs , thrice daily.

Yes its required, irrespective of vaccination status of the dog. Since blood has come, its category 3 exposure which will require inj tetanus toxoid + inj arv + inj rabies immunoglobulin asap & the injection schedule has to be adhered strictly. You did great by washing the wound with soap & water.

Firstly you only need 3 doses, no need for 4 dose.
No need to go for vaccination again. Just wash the area properly. And if you haven't done inj tetanus toxoid during 1st scratch than do it.
