Hi thanks for your query and welcome to lybrate. I am dr akshay from fortis hospital, new delhi. You can have shoulder pain at this age due to multiple reasons degenerative tears, joint arthrosis, adhesive capsulitis with restriction of movements, inflammation due to multiple reasons. So causes are many but if you are improving with physical therapy, then that is the way to go forwards and keep continuing with it at home as well, take appropriate calcium and vitamin d supplementation as required, and keep yourself physically fit. My wife, 65, a regular gym enthusiast, developed pain in the right shoulder which did not abate with any medication. A u. S scan indicated tendonitis. It is persisiting for the last 6 months. What is the remedy if there is one? is surgery an option? presently she applies topical gel and stretch band over the affected part. Your expert opinion please.

It seems you are suffering from frozen shoulder syndrome. Kindly show me rule out diabetes & vit. D deficiency or any other metabolic disorder. Do hot fomentation ibuprofen 200mg od & sos x 5days you will need other supportive medicines also. Do neck & shoulder exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. Surgery is usually not needed in such cases. But if at all it has to be done, you may come to delhi, we have some very good centers that look after this type of problems.

Taking Care of Your Joints as You Age

Skin is one of the most obvious parts of our body, which ages with an increase in the number of candles on our birthday cake. As you age, joints tend to become stiffer and less flexible. The body does not recover from bumps and bruises as easily as it did before, and it becomes difficult to stand for extended time intervals. This is caused mostly by a breakdown of cartilage and inflammation of the synovium.

What is cartilage?
Cartilage is a flexible connective tissue. It connects the ends of two bones in a joint. It is softer than bone tissue but less flexible than muscle tissue. Cartilage is responsible for the smooth movement of joints. 
With age, cartilage begins to degenerate. As the cartilage gets thinner, movement becomes more and more difficult. If left untreated, the cartilage may completely wear off leaving no buffer between the bones. This is one of the leading causes of knee replacement surgery. 

What is synovium?
Synovium is a membrane responsible for keeping the cartilage healthy. It produces a thick fluid that keeps the cartilage and joint lubricated. In cases of osteoarthritis, this membrane can swell up and produce excess fluid. This in turn causes the joint to swell as well. 

Along with age, factors like being overweight, genetics, gender and lifestyle also play a role in the condition of your joints. Being obese point towards a sedentary lifestyle and puts extra pressure on joints, leading to the thinning of cartilage. While men have higher chances of becoming osteoarthritis patients before 45, women have higher chances of developing this disease after 50. A healthy lifestyle is the easiest way to ensure that your joints work well even when you're old and grey. 

Exercise
Movement of the joints keeps the fluid in them moving. It also prevents the cartilage from shrinking and stiffening up. Exercise also makes the bones stronger, and slows the rate of bone tissue deterioration. Stretching exercises are also a good way to maintain the flexibility of joints. 

Healthy diet
Foods that are known as natural antihistamines, antioxidants and foods that fight inflammation promote healthy joints. Some of these are:

  1. Green tea: It prevents the breakdown of collagen and cartilage
  2. Nuts and seeds: These are rich in omega 3 fats that are essential for good bone health
  3. Ginger: It relieves pain and joint swelling
  4. Yogurt: It helps soothe tender joints and fights inflammation
  5. Pomegranate: It protects the cartilage against damage

Hot and cold therapy can also help relieve joint pain and reduce inflammation. As you get older, avoid putting too much pressure on your joints and give them adequate rest. Lastly, avoid smoking, as people who smoke have an increased risk of cartilage loss.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Since your history is only 4 days duration: my advice is: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication which suits you ( but avoid drinking when you have taken medication), also you can add a muscle relaxant for 5 days to relieve muscle spasm. - physical therapy initially under supervision of a trained physiotherapist and then to continue at home ( no exercises, only modalities initially, once your pain subsides, then we can start with gentle back strengthening ex) - adequate calcium & vitamin d intake if levels are low in body - ice packs for local application ( 3-4 times in a day) - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings and traveling on uneven surfaces especially on 2 wheelers take rest for 3-5 days so that pain and muscle spasm decreases. Calf pain after running is due to muscle cramps. Follow these to get relief and avoid the problem. 1. Consume plenty of liquids after and during runs. At least 1 litre of water for 1 hour of running. 2.Take healthy snack after running like fruits or oats or chicken or dal. 3 Take shower with cold water to relieve body soreness after runs. Apply ice pack on calf muscles after running. 4. Always do stretching of back and leg muscles before starting your run. 5. Take a break of at least one day a week . 6. Be gradual in your runs. Gradual increase in speed and distance of run is recommended. 7. Do not run with improper shoes. Wear good shoes with sole support. 8. Avoid running when pain is present. If problem persists consult again.

Its due to over straining. Do some stretching . Relax for sometime. The pain will go in a day's time. If the pain is unbearable accompanied by redness and swelling over the painful area then u must consult a doctor.

Hi if you have localized back pain with - no neurological symptoms like numbness, paresthesia, weakness in limbs etc, my advice is: - intensive physical therapy initially under supervision of a physiotherapist and then at home. - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know in a week time how you are feeling. I will actually ask you to get x ray of involved region of spine (ap & lateral) and send reports across to me so that I can advise further in a better manner. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

Hi, Fever may be simple/ self-limiting viral infection, or it may a symptom some serious illness like typhoid/ malaria/ Dengue/ TB or many more like that. It is decided by symptoms provided by you, and a battery of investigations. But short term fever is most likely viral fever. Therefore, follow the advice and consult doctor with detail symptoms. 1. Take rest for 1-2 days. Wear light and cotton cloths, it helps in cooling of body. 2. Take Tablet Paracetamol 500 mg (as and when required; after food) for fever (more than 99 F) for 3 days. Maximum 3 tablets with gap of 8 hours can be taken in a day. 3. If fever persists, do cold sponging (with cold water even ice water if needed) to lower down temperature. 4. Drink plenty of fluids. 5. Take adequate rest and proper sleep. Going to work, working out may just make you feel worse as the extra action adds stress to your body. Inadequate sleep decreases immunity of body, which causes worsening of illness. 6. Take proper diet preferably homemade like dalia, khichdi, rice, dahl, chapatti, vegetable soups or steamed veggies. 7. This is season of dengue, therefore, if you have high grade fever, and body ache, then have Platlet Count from nearby Laboratory. Consult again if symptoms persist or worsen. ***Get Well Soon*** “Cold sponging is the best treatment if fever is uncontrolled.”

Hand exercise dear, you can do it, 1……. Make a Fist Hand and finger exercises can help strengthen your hands and fingers, increase your range of motion, and give you pain relief. Stretch only until you feel tightness. You shouldn't feel pain. Start with this simple stretch: •Make a gentle fist, wrapping your thumb across your fingers. •Hold for 30 to 60 seconds. Release and spread your fingers wide. •Repeat with both hands at least four times. Finger Stretch Try this stretch to help with pain relief and to improve the range of motion in your hands: •Place your hand palm-down on a table or other flat surface. •Gently straighten your fingers as flat as you can against the surface without forcing your joints. •Hold for 30 to 60 seconds and then release. •Repeat at least four times with each hand. Claw Stretch This stretch helps improve the range of motion in your fingers. •Hold your hand out in front of you, palm facing you. •Bend your fingertips down to touch the base of each finger joint. Your hand should look a little like a claw. •Hold for 30 to 60 seconds and release. Repeat at least four times on each hand. Grip Strengthener This exercise can make it easier to open door knobs and hold things without dropping them. •Hold a soft ball in your palm and squeeze it as hard as you can. •Hold for a few seconds and release. •Repeat 10 to 15 times on each hand. Do this exercise two to three times a week, but rest your hands for 48 hours in between sessions. Don't do this exercise if your thumb joint is damaged. Apply prasarini oil or pranacharya restopain oil on affected part then give hot fomentation. Take maharasnadi kwath 2-2 tsf twice a day. 1-1 tab yograj guggul after lunch and dinner. Avoid tea. Coffee. Junk food. Spicy food. Fermented food. Take green vegetables. Drink juice. Eat fruits. Drink 7-8 glass of water in a day. Take 7 hours sleep minimum. Do pranayama early morning.
