You are seeking help and this itself is a step forward. Since you are on medication for a long time, please don't stop the medicines without consulting your doctor. Yes, medicines do have some side effects, but if you don't take these prescribed medicines, the result will be worse. You need psychotherapy. Kindly get in touch with a counsellor who will explore with you, the history of these attacks, what are the triggers now and how to manage it. You said you cannot go anywhere without medicines and" mother" what do you mean by that? are you not confidant to move around without your mother? it appears to me that you are underconfident and are not used to taking responsibilities. Your excess sleep and not eating food indicate stress. Your facial shock also might be the result of overall anxiety and low self esteem. Kindly see a counsellor. Please understand that we all have the ability to acquire" life skills" undergoing therapy and learning certain skills will certainly help you. All the best.

You must be able to distinguish between disappointment and depression. Disappointment shows many symptoms of depression and are short lived. Daily routines will help you over come them. I need to discuss this with you in detail. Post a private question to me. Take care.

Dear, you should be able to distinguish between disappointment and depression. Disappointment also brings on symptoms identical to depression but they are short lived. You went through a disappointing incident in the past and therefore you are feeling these symptoms. Don't worry. They are short lived. Secondly, don; t sit idle. Exercise, play games, study, learn, enhance your knowledge and get ready for a good career. Idle mind is devil's workshop, remember. Please post a private question to me with every detail. I will help you. Take care.

I am 24 years old male, engineer, suffering from neurocysticercosis and under minimum medicine dosage of levipil 250 twice daily. I came to Delhi for coaching and staying at my uncle's house along with his family. Mother is also staying with me only. Two days earlier there was a discussion or exaggeration regarding a topic on household activities was going among my cousins who are now matured parents and my aunt which indirectly hurt my sentiments very much but still I kept clam but I was not able to sleep that night I was feeling a hotness inside or can be said as sense of fever kind of thing. The next day another topic started but my uncle who likes criticising people did pinched me again with a sentence but still I didn't give any harsh reply but I just told him a very very practical reason that he couldn't continue with his superiority complex anymore. Next day he told my mom in my absence that he was hurt now as my mom conveyed me this still I am ready to apologize to him eventhough I have not done any mistake. But from yesterday night I again felt that heat sensation inside my body, a fever like feeling ears are hot headache behind and right side of head and eyes are red. Is it related to stress or may be some disease as dengue season is going on. Please help.

PhD (Psychology), MSc Psychology, MS
Psychologist,
I am 20 years old and I get easily irritated. Please help
Dear, anger and aggression are emotions. Anger comes when you become irritated. Irritation happens when you don't like something, or when something is repeated. As anger is an emotion, it should be vent out. You should be able to throw anger out instead of controlling it. But more perfect will be, know the frustrating situations and stop being emotional. If practiced properly, you will not get irritated at those circumstances at all. Those techniques are much easier to understand. Please post a private question with all the details and I shall help you with those tips and tricks. Take care.

My brother in law is a schizophrenia patient. After my marriage i came to know that my father in law too had faced some problem before his marriage (details not know), two more first cousins have had stints of depression. Is mental problem hereditary? What are the chances of my children having such problems? Are there tests to check if the child is ok before the delivery?

MD - Psychiatry, MBBS
Psychiatrist, Delhi
Yes. A proportion of mental disorders are hereditary but as it is not autosomal dominant or sex linked genetic disorder it can not be predicted surely that the child will have it or not. Yes, definitely the chances are more than the general population, in which the family history is negative. No in utero test can determine about a child to get mental illness in future. There is a two hit hypothesis proposed by scientists that genetic predisposition plus stressors in life can increase the chances of mental illness to occur.

You need to learn the" relaxation response" in times of stress and anxiety. This can be learnt in a step wise process only and with practice you can excel. Start with doing relaxation exercises like yoga and meditation. Also try breathing exercises for 15-20 minutes daily. Once you learn how to relax you will have to learn when to relax too. If you still find things difficult you might need help with some counselling and medications.

I am 55 years old. Suffering from depression/anxiety/bipolar disorder. Taking medicines since 1979. Have no pleasure in life. Also suicidal thoughts. What is the remedy? how to get better?

Reparenting Technique, BA, BEd
Psychologist, Bangalore
Yes, you will have suicidal thoughts but not if you were also working on these issues with a counselor. For all of these many years you have delayed getting better by failing to combine therapy with medicines. The challenges of everyday living pose a major stress to people and they learn to cope with it through various adaptations, techniques, and tools. In the case of the bipolar condition your adjustability is more difficult than the normal population. Apart from this there are many issues of the mind that medication cannot address, and these are ideally worked out in therapy. Please put yourself on a tight time-schedule with exercises, interesting activities, functional occupations; and being a contributor to your family and society at large. Eat healthily, sleep well, and go for regular counseling. There is a lot of anger that is not being dealt with for many years too. Suicide is not an option even though it may appeal to you because of your struggles. You can live a good life subject to your doing more in the manner explained above. Life will then look full of hope; and many of your desires will and can be fulfilled.

Why my friend fears from god, she think the god punish him. Please tell the solution.

MS - Counselling and Psychotherapy, MPHIL
Psychologist, Mumbai
Devotion or bhakti as we call it, is based on love, not fear. Where there is love there is god. Tell your friend not to fear god but to love him. God will never punish only guide. Like the bhagavad gita tells us" lord krishna who is dwelling in everybody's heart, acts as a best wishing friend and purifies the devotee who constantly engages in hearing him" tell your friend that when she says she is scared of god, essentially she is scared of her own self, as god is within every one of us. She need not be afraid of her most trusted friend and guide. Ofcourse if your friend has any questions or wants to talk about this at length she can always talk to someone who is open to this discussion or consult a counsellor and seek help.

You haven't mentioned why health issues you have but it seems to be stressing you out enough to impact your health. Thus stress and worry will not solve your health problems. If you want your health then you will need to do things that make you healthy. Problems are a part of everyone's lives but some people feel it more than others. This is because of the meanings and significance that they attach to these problems and their way of ruminating about it. Getting health checks done and taking medications timely is one way of taking care of your health. Another way is to exercise regularly, eat healthy food, reduce junk food and sleeping on time. Learning how to manage stress and tensions will help you remain healthy longer. A counsellor will be able to provide you with a non judgemental space where you can talk freely and openly about your problems and find workable solutions to your problems. Is some of the problems are not within your control, then learning to live with them amicably is one way out. Problems and mistakes are actually great teachers and if we have the right attitude can learn a lot and grow and develop personally and spiritually. A simple life without any complications would be very boring and unchallanging. Without experiencing sadness and disappointment how will you appreciate success, happiness and achievements? try to see life as a balance of positives and negatives and remember that bad times don't last forever. Good times will follow eventually and you will be up and about all over again. Being flexible and adaptable is a desirable trait in people who want to achieve a lot in life. These are general tips.

General Physician,
Quit smoking

Follow 5 As (Ask, Advise, Assess, Assist, Arrange)

Assessing tobacco use status during healthcare visits increases the likelihood of smoking-related discussions between patients and clinicians, as well as increases smoking cessation rates.
All smokers should be advised to quit smoking. Even brief advice to quit has been shown to increase quit rates.
For smokers who are willing to quit, manage them with combination of behavioral support and pharmacologic therapy
Behavioral intervention generally consists of brief clinician counseling in the office, but smokers should also be referred to a free telephone quit line
Smokers who fail to quit on an adequate trial of first-line pharmacotherapy may benefit from second line pharmacotherapy.
Also attempt to intensify behavioral counseling.
Patients who successfully quit but experience relapse be treated with a pharmacologic agent that previously helped the patient to achieve abstinence and was tolerable
Enhancing treatment by adding another pharmacologic agent or more intensive behavioral support can also be considered.
Smokers can also be advised about the health risks of secondhand smoke exposure to their household members and urged to commit to keeping their home and car smoke-free, especially if children are present.

5As of quitting Smoking

Ask: Implement an office wide system that ensures that, for every patient at every clinic visit, tobacco-use status is queried and documented. Repeated assessment is not necessary in the case of the adult who has never used tobacco or has not used tobacco for many years, and for whom this information is clearly documented in the medical record.

Advise: Strongly urge all tobacco users to quit in a clear, strong, personalized manner. Advice should be clear - "I think it is important for you to quit smoking now and I can help you" "Cutting down while you are ill is not enough" It should be "As your clinician, I need you to know that quitting smoking is the most important thing you can do to protect your health now and in the future. The clinic staff and I will help you" and it should be personalized. Tie tobacco use to current health/illness, and/or its social and economic costs, motivation level/readiness to quit, and/or the impact of tobacco use on children and others in the household.

Access: Determine the patient's willingness to quit smoking within the next 30 days; If the patient is willing to make a quit attempt at this time, provide assistance; If the patient will participate in an intensive treatment, deliver such a treatment or refer to an intensive intervention; If the patient clearly states he or she is unwilling to make a quit attempt at this time, provide a motivational intervention and If the patient is a member of a special population (adolescent, pregnant smoker), provide additional information specific to that population.

Assist: Provide aid for the person to quit.

Arrange: Schedule follow-up contact, either in person or by telephone. Follow-up contact should occur soon after the quit date, preferably during the first week. A second follow-up contact is recommended within the first month. Schedule further follow-up contacts as indicated.

Congratulate success during each follow-up. If tobacco use has occurred, review circumstances and elicit recommitment to total abstinence. Remind the person that a lapse can be used as a learning experience. Identify problems already encountered and anticipate challenges in the immediate future.

Research-based strategies to boost your memory and keep it strong 7 Strategies for Remembering 1.Become interested in what you're learning. We're all better remembering what interests us. Few people, for example, have a difficult time remembering the names of people they find attractive. If you're not intrinsically interested in what you're learning or trying to remember, you must find a way to become so. 2.Find a way to leverage your visual memory. You'll be surprised by how much more this will enable you to remember. For example, imagine you're at a party and are introduced to five people in quick succession. How can you quickly memorize their names? Pick out a single defining visual characteristic of each person and connect it to a visual representation of their name, preferably through an action of some kind. Remember: Memory is predominantly visual. 3.Create a mental memory tree. If you're trying to memorize a large number of facts, find a way to relate them in your mind visually with a memory tree. Construct big branches first, then leaves. Branches and leaves should carry labels that are personally meaningful to you in some way, and the organization of the facts (" leaves") should be logical. 4.Associate what you're trying to learn with what you already know. It seems the more mental connections we have to a piece of information, the more successful we'll be in remembering it. This is why using mnemonics. Write out the items to be memorized over and over and over. 5.When reading for retention, summarize each paragraph in the margin. This requires you to think about what you're reading, recycle it, and teach it to yourself again. Even take the concepts you're learning and reason forward with them; apply them to imagined novel situations, which creates more neural connections to reinforce the memory. 6.Do most of your studying in the afternoon. Though you may identify yourself as a" morning person" or" evening person" at least one Study. Suggests your ability to memorize isn't influenced as much by what time of day you perceive yourself to be most alert but by the time of day you actually study—afternoon appearing to be the best. 7.Get adequate sleep to consolidate and retain memories. Not just at night after you've studied but the day, you study as well. Far better to do this than to stay up cramming all night for an exam. 10 Study Tips to Improve Your Learning Study Tip 1: Underlining Underlining is one of the simplest and best known study tips. It’s easy to highlight the most significant parts of what you’re reading. One key sentence per paragraph and a few important phrases here and there. You can only retain a certain amount so it’s best to retain the most important information. Study Tip 2: Make your own note and taking notes is one of the most widespread study skills out there. Essentially the aim of note-taking is to summarise lectures or articles in your own words so you can easily remember the ideas. Study Tip 3: Mind mapping A good Mind map can save you many hours of study and further consolidate your knowledge for your exams. Mind Maps are an extremely versatile tools. They can be used for brainstorming, outlining essays or study topics and for general exam preparation, ExamTime offers the ability to create Mind Maps quickly and easily which makes them the ideal tool when it comes to exams. Study Tip 4: Flash cards:- Using these are a particularly effective method of learning when trying to assimilate different facts, dates, formulas or vocabulary. Subjects such as History, Physics, Maths, Chemistry, Geography or any language are made much easier if you incorporate Flashcards in to your study. Study Tip 5: Case Studies Sometimes it can be difficult to grasp the implications of some theories. This is where studying case studies can be a big help. Case studies can help you visualise a theory and place it in a more familiar and realistic context. This is especially useful in business or law subjects. Study Tip 6: Quizzes are an excellent way to review study notes in the weeks and days before an exam. Quizzes can show where your strengths and weaknesses are, so it allows you to focus your efforts more precisely. Moreover, if you share your Study Quiz with your classmates and test each other as much as possible you can discover even more details and areas you may have overlooked. So before any exam, make sure you create and share a bunch of different Quizzes with your Friends. Study Tip 7: Brainstorming This is another study technique that is ideal for studying with friends and/or classmates. Brainstorming is a great way to expand every possible idea out of any topic. Just get a bunch of friends together and shoot the breeze, there are no wrong answers when brainstorming – just talk and capture the ideas, you can review afterward. Study Tip 8: Mnemonic Rules Mnemonics are especially useful when memorising lists and sets. Mnemonics rules basically work by associating certain concepts with other concepts that are more familiar to us. There are many different ways to make mnemonics and these can be individual to the person. Study Tip 9: ORGANISE your study One of the most effective study skills is also one of the most often overlooked; this is organising your study. Creating a TIME TABLE gives you goals and a time in which to achieve them. Having a study timetable as you study is greatly motivational Study Tip 10: Drawing Many people find it easier to recall images rather than text that is why they are better able to memorise concepts if they associate them with pictures or drawings. If you find answer helpful please click on “helpful” tab for knowing my efforts are useful CONCENTRATION TIPS Here’s some tips to get into a state of deep concentration where work / or studies flow easily so that you can do well in examinations/ and or do your work well. 1) Cut Off the Noise Getting into a state of concentration can take at least fifteen minutes. If you are getting distracted every five, you can’t possibly focus entirely on your work. Request that people don’t interrupt you when working on a big project. If you are required to answer phones and drop-in’s immediately, schedule work when the office is less busy. 2) Structure Your Environment The place you work can have an impact on your ability to focus. Try to locate yourself so you are facing potential distractions such as doors, phones or windows. This way you can take a glance to assess sounds that would otherwise break your focus. 3) Clarify Objectives Know what your goal is clearly before you start. If you aren’t sure what the end result is, the confusion will make it impossible to focus. 4) Divide Blobs. If you have a large project that needs work, clearly identify a path that you will use to get started working on it. If the sequence of actions isn’t obvious, it will be difficult to concentrate. Taking a few minutes to plan not only your end result, but the order you will complete any steps, can save hours in wasted thinking. 5) Know the Rules Get clear on what the guidelines are for the task ahead. What level of quality do you need? What standards do you need to follow? What constraints are there? If the rules aren’t clear from the outset, you will slip out of concentration as you ponder them later. 6) Set a Deadline Deadlines have both advantages and disadvantages when trying to force concentration. A deadline can make it easier to forget the non-essential and speed up your working time. If you give yourself only an hour to design a logo, you will keep it simple and avoid fiddling with extravagant designs. Time limits have disadvantages when they cause you to worry about the time you have left instead of the task itself. 7) Break down Roadblocks Roadblocks occur whenever you hit a tricky problem in your work. This can happen when you run out of ideas or your focus wavers. Break down roadblocks by brainstorming or planning on a piece of paper. Writing out your thought processes can keep you focused even if you might become frustrated. 8) Isolate Yourself Become a hermit and stay away from other people if you want to get work done. Unless your work is based on other people they will only break your focus. Create a private space and refuse to talk to anyone until your work is finished. Put a sign on your door to steer away drop-ins and don’t answer your phone. 9) Healthy Body, Sharper Mind What you put into your body affects the way you concentrate. Nobody would expect peak performance if they showed up drunk to work. But if you allow yourself to get chronic sleep deprivation, overuse stimulants like caffeine or eat dense, fatty foods your concentration will suffer. Try to cut out one of your unhealthy habits for just thirty days to see if there is a difference in your energy levels. I’ve found even small steps can create dramatic changes in my ability to focus. 10) Be Patient Before I write an article, I often sit at my desk for a fifteen or twenty minutes before I put finger on the keyboard. During this time I feel a strong urge to leave or do something else. But I know that if I am patient, I’ll stumble upon an idea to write about and enter a state of flow. Without a little patience, you can’t take advantage of flow when it rushed through you. If you need strong concentration I recommend periods of 90-120 minutes. Any less than that and you will waste too much time getting started before the flow can continue. More than this is possible to sustain focus, but you will probably benefit from a quick break. These are the general guidelines for concentration, studying well or doing your job. Regards.
