I am 46 years old. I can't move my right hand freely. When I want to raise my hand then feeling pain in side the muscles near the shoulder. I din't take any medicine or exercise. Please give me a way to releif.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. This pain could be due to certain degenerative changes in your shoulder joint or its associated structures, or could be just muscular in nature. If there is no history of any injury, or any s/s of inflammation or infection like swelling, skin redness or local warmth etc, I will advise you initially to do: - ice packs at home 3-4 times in a day - gentle anti inflammatory medication like tab etoshine or tab paracetamol can be taken for 5-7 days or as sos basis - physical therapy with tens or laser under supervision of a physical therapist - analgesic spray like dynapar qps spray for local application - adequate calcium and vitamin d intake as per requirement. Let me know in 2-3 weeks time how you are feeling, otherwise we will get some investigations done for you. Do not hesitate to contact me if you need any further assistance.

I am having back ache. Above my lower back near the lungs area. I feel it sometimes while running but these days I think due to my sleeping posture the pain has increased a lot. But if I sleep properly nd take a hot shower nd target it in the pain area I get a relief instantly. Do I meed to be concerned or is this normal?

Lower back pain can be caused by a variety of problems with any parts of the complex, interconnected network of spinal muscles, nerves, bones, discs or tendons in the lumbar spine. Typical sources of low back pain include: The large nerve roots in the low back that go to the legs may be irritated The smaller nerves that supply the low back may be irritated The large paired lower back muscles (erector spinae) may be strained The bones, ligaments or joints may be damaged An intervertebral disc may be degenerating While back pain is extremely common, the symptoms and severity of back pain vary greatly. A simple lower back muscle strain might be excruciating enough to necessitate an emergency room visit, while a degenerating disc might cause only mild, intermittent discomfort. Identifying the symptoms and getting a diagnosis that pinpoints the underlying cause of the pain is the first step in obtaining effective pain relief. Treatment for lower back pain depends upon the patient's history and the type and severity of pain. The vast majority of lower back pain cases get better within four to six weeks, and lower back pain exercises are almost always part of a treatment plan. please take these medications as primary treatment plan, & have a thorough consultation with us as early as possible. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned back pain associated with radiating leg pain as your main symptom. If you have these pains with no other neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - you can also take medications for neuropathic symptoms like tab pregabalin or tab gabapentin in small doses for your radiating pain for around 2 weeks - physical therapy initially under supervision of a trained physiotherapist and then to continue at home initially focusing on swd/ift for back and tens for leg pain. - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. For pinched nerve/compressed nerve treatment varies, depending on the severity and cause of the nerve compression. You may find that you benefit greatly from simply resting the injured area and by avoiding any activities that tend to worsen your symptoms. You should see a doctor if you feel constant numbness or pain in the area despite minimal use or if the muscles in the affected area feel weaker over time. Seek immediate treatment if the symptoms are severe or the area feels cold or appears very pale or blue. You might consider seeing a physiotherapist. She/he can give you specialized stretches and exercises that aid in healing a pinched nerve. Certain exercises relieve the pressure on the pinched nerve, helping to reduce pain. Many of the stretches used in this healing process must be done with a trained professional, so don't do them on your own.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

I have regular pain in my left knee and carf. It is little bit moving. My age is 34 years. In my 18 my left heel was effected by leprosy infection. I took dot for two month. I left. And after some 5 years i took dots again complete course. This pain was emerged in month of ramzan. I was fasting all rozas. Till then this pain is regular. And in every ramzan it is severe. Medicine is not cure for everything. It seems to be rotator cuff tear. Treatment depends upon extent of tear. She will need proper evaluation and treatment. Till then, she can go for physiotherapy.

Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. If it does not give relief in 4-5days, contact me again.

If its paining alot, then you will require prescription medications for 3-4 days, take bed rest for 2 days, avoid lifting heavy weights, avoid bike ride hot formentations, apply oxalgin gel for local application, and follow up for medications,
