I have got Achilles Tendonitis (severe pain in my Achilles tendon at the back of left foot) due to car driving and using the clutch excessively, I have not had any injury. Could it be due to uric acid and a gout attack? I have had past history of uric acid? should I take Feburic along with Combiflam. I am 48 years old, weight is higher than normal.

Just use ice pack for 15 min over the painfull area. After relieving from pain try to do some stretching and strengthening exs. Ultrasonic therapy is also help with icepacks.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

My wife have back pain last 4- 5 years,Spin L4/L5 sacrelisation,doing daily morning related exercise.But now she complaining knee sweling and all body joints paining.Presently doc have given proteins and Calcium medicine consuming and as well as physiotherapy (TFT Traction,massage) from last one week. Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Do hot fomentation twice daily. Postural correction- always sit with a small pillow at your low back. Sit tall, walk tall. Extension exercises like bridging and bhujang asana x twice daily. Sleep on your back with a pillow under your knees. And get vit d levels checked. Go for regular walks follow 30-30 rule when at work. After every 30 min of work. Do 30 sec of stretching or stand and walk a bit.

I am 46 yes, diabetic but no medicine. 2.5 years back got injured during throwing stone to a man go tree by falling into the field. I got treatment from orthopedics ond take injection. But not completely cured, even pain in right shoulder, x-ray report OK. Exercise done regularly. Pls advice.

Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

My dad is having depression. He had cervical spine blockage, so he had laminectomy. Dad's hand shake when he take glass to drink something. Much a time, dad's hand shake when they are at rest. Usually in anxiety there will be shaking (tremors), but from what you describe it seems more like organic (neurological) rather than psychological. The leg jerks you state r likely to be myoclonic jerks, better consult a neurologist. All the best.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned back pain associated with radiating leg pain as your main symptom. If you have these pains with no other neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - you can also take medications for neuropathic symptoms like tab pregabalin or tab gabapentin in small doses for your radiating pain for around 2 weeks - physical therapy initially under supervision of a trained physiotherapist and then to continue at home initially focusing on swd/ift for back and tens for leg pain. - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation.
