Hello everybody, I'm dr. Pulkit Nandwani. I'm a consultant gynaecologist & laparoscopic surgeon at Saroj Medical Institute, Rohini. So if you are about to undergo any operation especially total laparoscopy hysterectomy for whatever the indication might be, this video would be very useful for you. Once an operation has been decided, the patient gets really scared because of the unknown factor. What will happen once you admitted in the hospital? What will happen after the operation? How will you go through it? Just knowing the basic details of the hospital and treatment will put you a lot at ease. So on the day when you're supposed to undergo total laparoscopy hysterectomy, you would be asked to come to the hospital for admission usually early morning with empty stomach. We might even ask you to have just a light diet the previous day or even liquid diet the previous day and then not take anything after that from twelve o'clock in the night and come stomach to the hospital in the morning with all your records, all the blood tests, the ultrasound reports, that would have been done previously when the operation would have been decided. Once you admitted in the hospital, a sister will be an incharge of you. You will be given a bed, you will be asked to change your dress, the area where we have to operate would be prepared by doing shaving of hair of that area and then and IV saline IV cannula will be inserted.

Following all the medications, all the glucose, all the IV fluids would be given through this cannula and you will not have any other pain except the cannula pain. After that, an anesthetist will come and have a word with you and have a pre-Anesthesia checkup will be done. Then at the scheduled time you will be shifted in the operation theatre. Once you're in the operation theatre, again through that IV cannula line itself medicines in the form of injections would be given. A mask would be put on your nose and you will be put to sleep. The whole operation which would be undertaken by a team of doctors along with a multitude of workers along with us technician, nurses and anesthetist. The whole procedure will take around one and a half hours to two hours and then post operations once we find that you're stable you would be shifted to the postoperative room where you would be kept there for another three to four hours just for extra monitoring.

Later you'll be shipped out to the room. After the operation, once you wake up you will just have little grogginess, heaviness of the head which would be for the after-effects of the anesthesia. You would not be allowed to eat at that time. All the nutrients for the body will be given By the IV fluids in the form of glucose. All the medicines in the form of injections would be given. There would be a catheter inside the urinary bladder to allow drainage of urine. This catheter we will remove most probably the next day morning itself. So we will start giving you food either in the evening or the next day morning. Liquids will also be started first and gradually the food will be started. You might be asked to get off the bed on that evening.

This is the advantage of a total laparoscopy hysterectomy. The next day you will be kept inside the hospital, but you would be eating properly you would be walking around the hospital just for observation and the amount of injectable antibiotics. You would be kept inside the hospital. The next to next day you will be discharged. We put the dissolvable sutures over that, we put waterproof dressing. So the very next day of the operation and throughout after that you are allowed to take bath, just the vaginal hygiene has to be maintained at home. Heavy weightlifting or bending a lot would be just advisable not to do but other than that routine procedures and home routine activity at home is allowed. For a checkup you will have to come after a week. I hope the video was informative for you and would put you at ease.

7 steps to prevent sperm leakage after intercourse

Many couples have complained that their inability to conceive is as a result of the sperm seeping out of the vagina after intercourse. Do you have a similar problem? Then read on.

This is an issue I will tackle objectively in this article. I have outlined some simple steps below that can be followed to reduce or if possible eliminate sperm leakages.

Make sure the vaginal canal is clean of any artificial lubricants as these slow down sperm and also encourage leakage of sperm.

Woman should pee (urinate) shortly before intercourse so that she can have time to relax after the show.

It is important that couples engage in foreplay so that the woman can reach orgasm especially when trying to be pregnant because the muscle spasm that goes along with orgasm can help pull the sperm into the uterus. Foreplay also helps the man to have a more exciting intercourse that promotes very powerful orgasm that will result in the production of a higher sperm count.

Be sure to get into positions that will permit the sperm to remain inside the vagina longer. The missionary style allows for deeper penetrations and allows sperm stay longer this is a highly advisable position to use. If it is discovered that the woman has tilted cervix then a rear entry is ideal for the man.

Some of the leakages are due to gravity. Avoid positions that allows for the sperm to leak out such as the woman being or top or some other positions where it is likely that sperm may leak out.

Time the thrust that will release the sperm and make sure that ejaculation occurs while the penis is deep inside. After ejaculation, the man should not stand up and remain till his member goes flaccid after which he can rise up.

The woman should place a pillow under her hips to elevate her pelvis. And should remain like that for at least 30 minutes. This will allow the sperm have a enough time to swim downwards into the cervix rather than leaking out.

Honestly you would have reduced sperm leakages by 90% if you attach importance to this article. Try the 7 steps above and you would have taken a bold step to improving your chances of conceiving this month.

Try conceiving during fertile days of your cycle i. E. Ovulation days. Get basic tests of you done i. E. Amh tsh fsh prolactin and ultrasound pelvis and husband semen analysis done. Also start doing regular exercise, it will enhance fertility. Take nutritious diet with fruits juices and salad.

She can take normal home made food I am giving you a brief diet chart breakfast: never skip breakfast, I am giving you few option of food items you can take 2-3 toast + 1 bowl of milk with cereal or a bowl oats/porridge or parathas/upma/poha mid morning / snack: 1 fruit + yoghurt, you can make fruit raita + nuts or fruit juice lunch: 2-3 rotis + vegetables + 1 bowl pulses (dal) +add a bowl of yoghurt or paneer + 1 bowl of rice / paneer sandwich + salad evening snack: vegetable /sandwich can add cheese / night dinner: same as lunch can avoid rice.

Ovarian Cysts - Know The Common Types!

Ovaries are a part of a woman’s reproductive system. The primary function of ovaries includes producing ‘ova’ or eggs and secreting hormones such as progesterone and estrogen. Cysts are fluid-filled sacs that form the ovaries; they usually do not cause any symptom and are not painful.

There are primarily two types of ovarian cysts:

1. Follicle cysts: During a woman’s menstrual cycle, the egg develops in a sac known as the follicle. Under normal circumstances, the sac breaks open and releases the egg. When this doesn’t happen, fluids start accumulating in the follicle to form a cyst.

2. Corpus luteum cysts: Follicle sacs dissolve after releasing the egg, but in some cases, these sacs remain and the opening of the sacs gets sealed. It again results in fluid accumulation, leading to the formation of corpus luteum cysts.

Symptoms

Usually, cysts do not cause any symptom. If the size of the cysts increases, they may cause symptoms such as stomach pain, pain during bowel movements and sex as well as pelvic floor pain. The breasts may become tender and one may experience rapid breathing. Other symptoms of ovarian cysts are fever, nausea and dizziness. Usually, rupturing of a cyst leads to these symptoms surfacing; hence you would know when exactly to call the doctor.

Treatment

The treatment options for ovarian cysts are:

  1. LaparoscopyLaparoscopy is carried out if the cysts are small in size. An incision is made close to the navel, through which an instrument is inserted to get rid of the cyst.
  2. Birth control pills: For chronic ovarian cysts, oral contraceptives are prescribed to stop the ovulation process in order to arrest the formation of cysts.
  3. Laparotomy: In case of large cysts, this procedure is recommended. A relatively bigger incision is made in the abdomen, through which the cyst is removed.

Ovarian cysts, if left untreated, can certainly cause infertility. Pre-menopausal women and who suffer from frequent hormonal imbalances in the body are the most vulnerable to this condition.

only diet walk exercise and wt red can control ur pds ushd remove stressand keep in mind that pcos can only be corrected it cannot be cured

This can be due to infection so better visit a doctor for examination. Also if periods are missed then get urine pregnancy test done.
