Diagnosis and Management of Asymptomatic Neoplastic Pancreatic Cysts

MBBS, MD - General Medicine, DM - Gastroenterology
Gastroenterologist, Delhi
Diagnosis and Management of Asymptomatic Neoplastic Pancreatic Cysts

The diagnosis as well as management of pancreatic cystic lesions is a general problem. Nearly 1% of the patients in the chief medical centers have been observed to have pancreatic cystic lesions on cross sectional imaging. It has also been observed that a quarter of all pancreas scanned in an autopsy series contain pancreatic cysts. Earlier, these cystic lesions were regarded benign but with increasing evidence made available from the cystic lesions, they are regarded as origin of pancreatic malignancies.

Information on Asymptomatic Neoplastic Pancreatic Cysts: The most vital medical tools that are used in the diagnosis and management of pancreatic cystic lesions include the endoscopic ultrasound and cross sectional imaging. These are used to distinguish non-mucinous cysts from mucinous cysts. The identification of pancreatic cysts creates a lot of anxiety for the clinicians as well as the patients related to the probable presence of a fatal tumor. The findings of a macro cystic lesion that enclose viscous fluid loaded in CEA are helpful in the analysis of a mucinous lesion.

The most common pancreatic cysts are the non-neoplastic inflammatory pseudo cysts, and they can be detected easily by imaging. The identification of pancreatic irregularity with probable association with malignant cells is a vital source of referral for the specialist. The set of guidelines that have been proposed for the management and diagnosis of patients with asymptomatic neoplastic pancreatic cysts are based specifically on the analysis of the quality of the data. It is also designed to address the most important and frequent clinical scenarios. The diagnostic suggestions are provided based on the clinical problem as well as the risk of malignancy.

Imperative Guidelines to Follow: To achieve accurate diagnosis of asymptomatic neoplastic pancreatic cysts is indeed a great challenge. It is all the more important to find the reproducible methods that can be used to stratify threat of cancer for the patients. The main guidelines include a two year screening interval of cysts that can be of any size as well as stopping observation after 5 years, in case there is no change.  The new guidelines, for the most part, recommend surgery if more than one concerning feature is confirmed on the MRI by use of endoscopic ultrasound. The new guidelines even suggest discontinuation of inspection after the surgery if no dysplasia or invasive cancer is identified. The guidelines have mainly been developed by use of Grading of Recomendations Assessment, Development and Evaluation.

I have keloid on my ear and also itching problem on it and it is increased day by day. It looks so ugly on my face. How can be it flat and how can I get rid from keloid.

MCh Plastic Surgery, MS - General Surgery
Cosmetic/Plastic Surgeon, Bangalore
Hello, There are various options for keloids from silicone gel creams, intralesional steroid, 5Fu Botox, Cryo surgery etc. First you can start with intra lesional steroid injection once in 3 wks you may need 4-6 sessions for it to become flat. What you need to understand is there is no cure it only control. Once its flat with treatment you have to observe and repeat treatment when it starts to come back. Surgery when done alone has more than 70% chance of recurrance and infact can become bigger than previous one. So its important to Do intralesional excision and give steroid after surgery. Consult a plastic surgeon.

Hi, lybrate-user Obesity or being overweight is due to metabolic imbalance in our body. Due to excess consumption of carbohydrate. Body turns in to fat deposition machine. Less the physical activity and more carbohydrate intake is the main reason behind the extensive weight gain. You can turn your body into fat burning machine by reducing intake of carbohydrate in diet. So eat less of carbohydrate like grain, wheat, rice and sugar. Eat more of vegetable, protein ,and fat. To improve your metabolism also take. Lipolyzer tummy tab. After lunch and after dinner. Microfruit tab. After breakfast replace your white salt with mineral salt.

Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance... if you keep in touch personally.

You can do sex to the fullest with the foreskin. Wash the area inside the skin &your head of penis retracting the skin daily while taking bath as smegma (solid white semen) deposits there. If any problem in future the option of circumcision operation is always there.

See till now Psoriasis is not having That medicine which cures it fully. So one ans is no you cnt get rid of it. I thnk you need to understand the disease first. In this disease the the time period of cell production decreases to 4 days and the scales which you r seeing is just your dead skin cells with redness (inflamation ). This is in short for the disease. With allopathic medicines you can control it but can not cure it fully. But yes being a Allopathic Dr. I saw some ppl getting benifitial results from homeopathic treatment. But yes a good, trained and experienced homeopathic Dr. Shud be the one to treat you. I hope this clear your doubt somehow. Do not hesitate to ask any further question. You can ask question privately too if you hesitate here. Regards. Dr. Sushant.
