With proper management of Diet one can control diabetes but cannot remove it altogether. To help manage your diabetes, your meals need to be: • Regular and spread evenly throughout the day • Lower in fat, particularly saturated fat • If you take insulin or diabetes tablets, you may need to have between meal snacks Matching the amount of food you eat with the amount you burn up each day is important. Not putting too much fuel in your body (keeping food intake to moderate serves) is vital to getting the right balance. Along with healthy eating, regular physical activity can help you to manage your blood glucose levels, reduce your blood fats (cholesterol and triglycerides) and maintain a healthy weight. . It is important to recognise that one diet does not fit everyone with diabetes. So my slogan for all diabetics is 'One Diet does Not Fit All'. So every individual diabetic should consult a dietician to plan a way according to individual body,tastes and requirements.Only then can you control your blood sugar levels. Fat Fats have the highest energy (kilocalorie) content of all foods. Eating too much fat can make you put on weight, which may make it more difficult to manage blood glucose levels. While it is important to try and reduce fat in your diet, especially if you are trying to lose weight, some fat is good for health. Saturated fat It is important to limit saturated fat because it raises your LDL (‘bad’) cholesterol levels. Saturated fat is found in animal foods like fatty meat, milk, butter and cheese. Vegetable fats that are saturated include palm oil (found in solid cooking fats, snack foods or convenience foods) and coconut products such as copha, coconut milk or cream. To reduce saturated fat: • Choose reduced or low-fat milk, yoghurt, ice-cream and custard • Choose lean meat and trim any fat off before cooking • Remove the skin from chicken (where possible, before cooking) • Avoid using butter, cream, sour cream, coconut milk, coconut cream and hard cooking margarines • Limit the amount of cheese you eat and try reduced-fat and low-fat varieties • Limit pastries, cakes, puddings, chocolate and cream biscuits to special occasions • Limit pre-packaged biscuits, packet snacks, cakes, frozen and convenience meals • Limit the use of processed deli meats ( chicken loaf, salami etc) and sausages • Avoid fried takeaway foods such as chips, fried chicken and battered fish and choose BBQ chicken (without the skin) and grilled fish instead • Avoid pies, sausage rolls and pastries • Rather than creamy sauces or dressings, choose those that are based on tomato, soy or other low fat ingredients. As some tomato and soy sauces can be high in salt, choose low-salt varieties or make them yourself without any added salt. • Limit creamy style soups. Polyunsaturated & monounsaturated fats Eating small amounts of polyunsaturated and monounsaturated fats can help ensure you get the essential fatty acids and vitamins your body needs. Polyunsaturated fats include: • Polyunsaturated margarines (check the label for the word ‘polyunsaturated’) • Sunflower, safflower, soybean, corn, cottonseed, grapeseed and sesame oils • The fat found in oily fish such as herring, mackerel, sardine, salmon and tuna. Monounsaturated fats include: • Canola® and olive oils • Some margarines • Avocado. Seeds, nuts, nut spreads and peanut oil contain a combination of polyunsaturated and monounsaturated fat. Ideas for enjoying healthy fats • Stir-fry meat and vegetables in a little canola® oil (or oil spray) with garlic or chilli • Dress a salad or steamed vegetables with a little olive oil and lemon juice or vinegar • Sprinkle sesame seeds on steamed vegetables • Use linseed bread and spread a little canola margarine • Snack on a handful of unsalted nuts, or add some to a stir-fry or salad • Spread avocado on sandwiches and toast, or add to a salad • Eat more fish (at least three times a week) because it contains a special type of fat (omega-3) that is good for your heart. • Do more dry roasting, grilling, microwaving and stir-frying in a non-stick pan • Avoid deep fried, battered and crumbed foods Carbohydrate I recommend to eat a moderate amount of carbohydrates for all Diabetics means, on a plate, approx, 1/4 of the plate should be carbohydrate foods. Carbohydrate foods are the best energy source for your body. When they are digested they break down to form glucose in the bloodstream. Insulin takes the glucose out of the blood and deposits it into the muscles, liver and other cells in the body where it is used to provide energy. A regular carbohydrate intake is required to provide our body and brain with instant energy. Most foods contain carbohydrate and also provide us with fibre, vitamins and minerals. Very low carbohydrate diets are not recommended for people with diabetes. If you eat regular meals and spread your carbohydrate foods evenly throughout the day, you will help maintain your energy levels without causing large rises in your blood glucose levels. If you take insulin or diabetes tablets, you may need to have between meal snacks. Discuss this with your doctor or dietician. All carbohydrate foods are digested to produce glucose but they do so at different rates – some slow, some fast. The glycemic index or GI is a way of describing how a carbohydrate containing food affects blood glucose levels. The type of carbohydrate you eat is very important as some can cause higher blood glucose after eating. The best combination is to eat moderate amounts of carbohydrate and include high fibre foods that also have a low GI. Sugar A healthy eating plan for diabetes can include some sugar. However, it is important to consider the nutritional value of the foods you eat. In general, foods with added sugars should be consumed sparingly (manufacturers sometimes use fruit juice or other sources of sugar to avoid using table sugar). In particular, high energy foods such as sweets, lollies and standard soft drinks should not be consumed on a regular basis. Some sugar may also be used in cooking and many recipes can be modified to use less than the amount stated or substituted with an alternative sweetener. Select recipes that are low in fat (particularly saturated fat) and contain some fibre. Alternative sweeteners The use of intense sweeteners by people with diabetes is preferable to use of natural sugars. Protein Choose protein foods that are also low in fat. This will help to reduce the amount of saturated fat you eat. Protein foods include lean meat, poultry without the skin, seafood, eggs (not fried), unsalted nuts, soy products such as tofu and pulses (dried beans and lentils). Herbs, condiments & drinks You can use these following foods nd add flavour and variety to your meals. • Herbs, spices, garlic, chilli, lemon juice, vinegar and other seasonings. • Products labelled ‘low calorie’ (e.g. low calorie/diet soft drinks, low calorie jelly). • Tea, coffee, herbal tea, water, soda water, plain mineral water. • Limit your alcohol intake if you choose to drink

I am 24 years old. Now my thyroid report is here TOTAL TRIIODOTHYRONINE (T3) =122 ng/dl TOTAL THYROXINE (T4) C.L.I.A 7.4 μg/dl TSH level is 24.54 μIU/ml. What can I do. I am very afraid about my TSH level.

Do no cabbage, broccoli, no radish, mustard, turnip, soya, no fatty / fast / spicy / processed food. Less tea / coffee. Take more water repeat tsh after one month and report good luck.

I am 46 years old & I am suffering from backache, uric acid & throid from six months.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned backache as your problem. If you have only localized backache with no other associated symptoms like radiating leg pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc there are ways to keep your uric acid value in body to low levels. General recommendations are: - decreasing intake of red meat, alcohol, rice in your diet and other purine rich sources - also increase your water intake so that uric acid is excreted from body well. - keeping your weight to normal levels will also help you. - intake of fresh fruits, citrus fruits and cherries has been implicated in reducing uric acid values in the body as well. Nowadays there is a medication by the name of febuxostat which helps in lowering uric acid levels in body but kindly check out that it suits your body. Urinary incontinence, commonly known as loss of bladder control can really cause embarrassment. The severity of this condition may range from once in a while leaky urine to the sudden and forceful urge to urinate, due to which you end up urinating before reaching the toilet.

Types of Urinary Incontinence:

  1. Stress Incontinence: When you feel the urge to urinate due to sneezing, coughing, laughing or while doing physical exercise
  2. Urge Incontinence: When you feel the urge to urinate, suddenly. This may occur during the day and even at night.
  3. Functional Incontinence: When you cannot reach the toilet on time because of a physical or mental impairment.
  4. Mixed Incontinence: When you involuntarily urinate due to more than one incontinence

Stress incontinence is the most common type in women. The activities which increase your risk, are:

You have had more than one pregnancy and vaginal delivery.

You have pelvic prolapse. This is when your bladder, urethra, or rectum slide into the vagina. Delivering a baby can cause nerve or tissue damage in the pelvic area. This can lead to pelvic prolapse months or years after delivery.

Female urinary stress incontinence is the involuntary release of urine during any physical activity that puts pressure on your bladder. This potentially embarrassing condition differs from general incontinence in that it happens when the body is under immediate physical stress. Activities that can put stress on your bladder include coughing, sneezing, laughing, lifting heavy objects, or bending over. This condition is particular to women, many of whom experience symptoms after muscles have been weakened due to a vaginal childbirth, following menopause, or during pregnancy.

Ensure that you consult a doctor:

  1. If this condition hinders your day to day activities
  2. If the urinary incontinence is the result of a serious underlying problem

Causes of Urinary Incontinence:

Urinary incontinence may be temporary or permanent. Causes for temporary incontinence are-

  1. Alcohol

  3. Decaffeinated tea or coffee
  4. Spicy and sugary foods
  5. High intake of Vitamin B or C
  6. Urinary tract infection (UTI)
  7. Constipation

  5. Hysterectomy

Chronic urinary incontinence can lead to certain complications like:

  1. Skin Problems: Skin rashes and infections can develop around the vaginal area, because of the constant wetness one feels. It may also cause sores.
  2. Urinary Tract Infections: Urinary incontinence can also result in UTI
  3. Impact on Personal and Social Life: Urinary incontinence can lead to great embarrassment, thus affecting the social and personal life of an individual.

Treatment for Female Stress Urinary Incontinence:

There are several types of treatment available. Treatment options include lifestyle changes, medications, nonsurgical treatments, and surgeries.

  1. Lifestyle Changes: Make regular trips to the restroom to reduce the chance of urine leakage. Drink fewer fluids and avoid activities such as jumping and running.
  2. Medications: Medications that reduce bladder contractions.
  3. Nonsurgical Treatments: Kegel exercises strengthen your pelvic muscles. Done regularly, these exercises can strengthen your muscles, allowing you to control when urine is released from your body.


If other treatments fail, you will have to go for surgical treatment. Types of surgery include:

  1. injectable therapy, in which collagen is injected into your urethra to reduce incontinence
  2. tension-free vaginal tape (TVT) surgery, in which mesh is placed around the urethra to give it support
  3. vaginal sling surgery, in which a sling is placed around the urethra to support it
  4. anterior or para-vaginal vaginal repair surgery to repair a bladder that is bulging into the vaginal canal
  5. retropubic suspension surgery to move the bladder and urethra back into their normal positions

Some doctors even try electrical stimulation and medication. When the condition is highly advanced interventional therapies and surgery is recommended.

Some interventional therapies are:

  1. Bulking material injections
  2. Botulinum toxin Type-A
  3. Nerve stimulators

Dear lybrate-user, Please give more details of your symptoms, problems you are facing, what consultation you have done, with a psychiatrist or any other doctor, what tablet was given, how much dose, for how long, what results do you expect. OCD is a long term treatment since it is a difficult condition to treat. The first consultation will be diagnosis and assessment. Later if you are put on tablets, they need periodic (every 2-3 weeks) to assess and increase the dose or add another drug of therapy. Therapy with ERP also takes many sessions depending on the number of obsessions you have. The result you expect is how to live with these obsessions without problems in your day today life. Please keep meeting the psychiatrist as often as she/he asks you to meet.

I have been feeling very weak in the recent past. I have also been having stomach ache ND other weakness for the past 2 months. I'm not able to pass stool properly for the past 2 months either. Even if I do, I take min 40 min and evn after that feel full. I also hv noticed blood in stool. When enquired with a doctor she thought it to be hemorrhoids ND have medicines. Evn after having such medication I still continue to v the same problems.
