I have been diagnosed with spiking of tibial spine bilaterlly, tiny marginal ostophytes formation seen at right patellar polar region. I feel some hard object while lying down at night and also burning sensation in my anus region. Pl advise?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Spiking of tibial spine with marginal osteophytes are indication of degenerative changes in your knee region. Treatment of osteoarthrosis depends upon what grade of degeneration has occurred, what is your knee range of movements and what medications like analgesics do you use on a daily basis? recommendations for early oa are: - start doing daily brisk walks for at least 2-3 km in a day in mornings. - intensive knee - quadriceps and hamstring strengthening exercises should be initiated initially in presence of a trained physical therapist and then at home on your own - avoid sitting cross legged, not to use indian style washrooms, do not do anything which eccentrically loads the knee and increases cartilage breakdown. - adequate calcium and vitamin d metabolism in terms of appropriate intake is important to keep bone metabolism ok - you can take short course of anti inflammatory medication like tab etoshine on sos basis especially when there is an episode of inflammatory pain, swelling etc. Do not hesitate to contact me if you need any further assistance.

Ice therapy would definitely help to reduce the inflammation. We also advise you to use knee cap which would help to prevent the knee from damaging further and also to maintain the quadriceps muscle tone. Simple Knee Exercises would help to strengthen the quadriceps muscle which is the key muscle. Specific knee exercises will also help ie. Keeping a soft ball underneath the knee and you have to keep pressing it. That's the simple exercise which will help you to strengthen the knee. We also advise you to use knee cap which would help to prevent the knee from damaging further and also to maintain.

Hand exercise dear, you can do it, 1……. Make a Fist Hand and finger exercises can help strengthen your hands and fingers, increase your range of motion, and give you pain relief. Stretch only until you feel tightness. You shouldn't feel pain. Start with this simple stretch: •Make a gentle fist, wrapping your thumb across your fingers. •Hold for 30 to 60 seconds. Release and spread your fingers wide. •Repeat with both hands at least four times. Finger Stretch Try this stretch to help with pain relief and to improve the range of motion in your hands: •Place your hand palm-down on a table or other flat surface. •Gently straighten your fingers as flat as you can against the surface without forcing your joints. •Hold for 30 to 60 seconds and then release. •Repeat at least four times with each hand. Claw Stretch This stretch helps improve the range of motion in your fingers. •Hold your hand out in front of you, palm facing you. •Bend your fingertips down to touch the base of each finger joint. Your hand should look a little like a claw. •Hold for 30 to 60 seconds and release. Repeat at least four times on each hand. Grip Strengthener This exercise can make it easier to open door knobs and hold things without dropping them. •Hold a soft ball in your palm and squeeze it as hard as you can. •Hold for a few seconds and release. •Repeat 10 to 15 times on each hand. Do this exercise two to three times a week, but rest your hands for 48 hours in between sessions. Don't do this exercise if your thumb joint is damaged. Neck stiffness and pain. Headache that may originate in the neck. Pain in the shoulder or arms. Inability to fully turn the head or bend the neck, sometimes interfering with driving. Grinding noise or sensation when the neck is turned. It requires both exercises and medication.

I am 52 years old and had a angioplasty in march 2010 and on regular follow up now same doctor has advised for angiography. Further I have cervical spondolysis. Head always remain heavy. Please advise what to do?

Don't worry. Took physiotheraphy for cervical spondylosis. And avoid pillow while lyinig. Use collar for neck frequently. And take tab. Shelcall d3 one a day for a month with the advice of your cardio doc.
