All About Cochlear Implants

MBBS, MS - ENT ( Gold Medalist Honors )

All About Cochlear Implants

A cochlear implant is beneficial if you are deaf or have difficulties in hearing. Cochlear implants are not hearing aids which just raise the volume of the different sounds you hear; they are surgically implanted inside your head. The implant transmits impulses straight to the auditory nerves which send the signals to the brain.

Cochlear implants can help you with sounds such as telephones, alarms or doorbells ringing. You can even understand speech or listen to music again with the help of cochlear implants.

How Cochlear Implants Work?

Cochlear implants are for adults and children with sensorineural (hearing loss caused by damage to the inner ear) hearing loss.

The device consists of two parts:

  • The receiver-stimulator: this part is surgically implanted under the skin.

  • The speech processor: this part is placed externally, behind the ear.

The receiver-stimulator is placed first, and two weeks after the procedure, the speech-processor is installed. If there are sounds around you, the microphone on the speech processor will pick them up and transform them into electrical impulses. The receiver-stimulator receives these impulses through the electrodes inside them. The electrodes then send the electrical impulses to stimulate the auditory nerves, and ultimately, the sounds picked up will reach the brain.

Advantages Of Cochlear Implants-

Some advantages of cochlear implants include:

  1. Being able to hear at normal volumes.

  2. Understanding speech without the need to lip-read.

  3. Listening to and talking over the phone get easier.

  4. Being able to hear music better after the surgery.

  5. Picking up loud, medium and soft sounds.

  6. Modulating your own voice gets better, so others understand you easily.

Disadvantages/Risks Associated With Cochlear Implants-

The risks of cochlear implants are common surgical after-effects such as infection, bleeding and anaesthetic side-effects. Some other complications include:

  1. Alteration in sense of taste due to nerve injury.

  2. Paralysis or weakness on the face due to nerve damage.

  3. Dizziness.

  4. Tinnitus (Ringing in the ear).

  5. Fluid leakage around the brain.

  6. Device can be faulty or get infected.

  7. Meningitis (An infection in the brain).

People Who Can Get Cochlear Implant-

You are eligible to get a cochlear implant if you have:

  1. Profound to severe hearing loss.

  2. Problems with the hearing aid.

  3. A strong wish to improve your hearing.

  4. Even children with severe hearing loss in both the ears can get a cochlear implant. If you wish to discuss about any specific problem, you can consult a specilized ear-nose-throat-ent-specialist and ask a free question.

7 Tips To Protect Your Hearing!

7 Tips To Protect Your Hearing!

Your hearing is one of the most important function your body carries out and sometimes you realize that you are losing your sense of hearing, when it is too late to get it back. There are two main reasons for this. One of the reasons is age and the other is when the inner ear's hair cells break down and do not pick up sound well.

Here are 7 ways to safeguard your hearing:

  1. Try to avoid loud places: It is not advisable to go to places where you have to shout to be heard, such as in a street, a concert or a construction site.
  2. Buy low noise rating equipment: The equipments in your house will make sound that you hear the most. Try to avoid these equipments by buying appliances with a low noise rating.
  3. Wear hearing protection at loud places: However, it is true that it is not always possible to avoid loud noises. This is when you need to get hearing protection. Earplugs and earmuffs are two of the best ways to make sure that even when you are in a loud place, your hearing does not get affected. Earplugs and earmuffs generally reduce sound by 15 to 30 decibels, which may be crucial to make sure that later in your life, you do not lose your hearing.
  4. Avoid smokeSmoking raises your chance of hearing loss. Second hand smoke does the same thing. Therefore, try to avoid, both smoking and secondhand smoking.
  5. Remove earwax properly: Earwax cannot be removed properly using a cotton swab. Instead, you should use an irrigation kit. Remember this as otherwise; the earwax could muffle your hearing.
  6. Avoid medications which reduce hearing: Certain medications increase hearing loss. Therefore, double check with your doctor to make sure your medicines will not make you lose your hearing.
  7. Get your hearing tested: Finally, get your hearing tested as identifying the problem early on can help stop worsen the situation.

Try to avoid usage of headphones. Coz by doing that you r putting the sound source close to tympanic membrane. Excess use may cause pain. And the ringing sound is called tinnitus. Some how that is also related to this. So go for otoscopic examination, puretone audiometry, impedance audiometry test. Share the reports with me. If possible consult e. N. T specialist available nearby.

My husband says that if he can sing songs and recite dialongs /songs there should not be a problem with the hearing. ...
Dear Lybrate User, There are many individuals who can not hear that well, but still can sing song and recite dialogues or songs. There is no one to one correlation. Yes, hearing is required learn singing and exactly copying the dialogue with tone. However, there are other symptoms of not hearing. If you talking about kids who may have hearing problem, then you should see if they respond to name calls, understanding what you say without looking at your face, or asking for repetition. If you think these symptoms are present, then you should get the hearing checked.

My father lost hearing almost 90% all of a sudden at the age of 86 years. The doctor advised that the loss of hearing is due to destruction of nerves due to old age. Is this curable or else hearing aid is required to be used. What is the correct rating and make of the hearing aid that can be used?

Master of Hospital Administration, Bachelor of Audiology & Speech Language Pathology (B.A.S.L.P)

Most probably its age related and your father has a sensorineural type of hearing loss. Sometimes steroids and multivitamins works in young age tht is also very little. At this age (86) hearing aid is a must without any doubt.

Conductive Hearing Loss - How It Can Be Treated?

Conductive Hearing Loss - How It Can Be Treated?

The ear has three parts – outer, middle, and inner – through which sounds waves pass through. On reaching the inner ear, the sound waves are transmitted through a fluid medium which is carried through to the brain to produce “sound.” Hearing loss can be due to problems anywhere in this circuit – from the outer ear to nerve issues in the middle ear.

There are two main types of hearing loss
Conductive hearing loss, where problems in the outer or middle ear cause problem conducting sound waves into the inner ear
Sensorineural hearing loss, where there is a problem in the nervous portion of the hearing process

Causes

Conductive hearing loss occurs due to poor conduction of sound waves to the middle ear, where it is processed to perceive the sound. Conductive hearing loss can happen due to the following reasons:

  1. Wax: Excessive wax accumulated in the outer ear can prevent passage of sound into the middle ear
  2. Infection: The outer or middle ear could be infected, affecting passage of sound waves
  3. Tear in the eardrum: The eardrum is a membrane through which sound waves pass. A tear due to trauma or injury can hamper this process, leading to hearing loss.
  4. Impact: When there is a bang on the head, there could be temporary hearing loss due to disruption of the hearing system
  5. Hereditary: A lot of times, hearing loss is hereditary and an exact cause may not be traceable
  6. Ear tumor: Middle ear tumors can also cause conductive hearing loss
  7. Abnormal growth: Abnormal bone growth in the middle ear can cause blockage, leading to hearing loss
  8. Post-surgical: The ear is a highly complex structure, and while surgical corrections are being done to treat a condition, it is very likely that other parts can be damaged

Symptoms

In most of the conditions mentioned above, there are some sound waves which pass through, so conductive hearing loss is often partial, wherein the person can hear but it is faint. Very rarely, it is complete where the person goes completely deaf.

Management

This would depend on the cause of the hearing loss.

  1. Clearing up the wax can lead to improved hearing
  2. A tear in the eardrum or fluid buildup would need surgical repair to restore hearing
  3. Infections might require treatment with antibiotics and pain killers
  4. Tumors or bone growths would also require surgical correction for restoring hearing
  5. For outer ear blocks, audiometry is done only after the blocks are cleared
  6. If post correction, the hearing is not completely restored, hearing aids can be used, which often results in complete restoration. If you wish to discuss about any specific problem, you can consult an Ent Specialist.

Dear Lybrate User....before Stapedectomy you can try Homoepathic THIOSINAMAN 200, 5 drops in a cup of water early morning before brushing your teeth along with Homoeopathic FERRUM PIC 6X tablets, 4 tablets, thrice daily for at least 1 month....this combination has effectively increased the level of hearing in some cases of deafness due to otosclerosis in my experience
