I have pimples and black spots on my face and they come back every week. What should I do?
Use tea tree face wash and toner twice daily. Use aha glow face wash daily use sunscreen with spf 30 daily. Use natural face pack of multani mitti and lemon 2 times a week. You can also use apple cider vinegar with mother for 5 mins after washing your face and splash it clean with water. Most important is diet- do not eat junk, eat more vegetables and fruits especially with vitamin c.

Age 29 years I had a knee pain in right leg. After mri ,doctor told that it is osteochondral defect and suggest for immediate orthoscopy I consulted another doctor, he suggested for physiotherapy, Now my pain and problem is 60% reduced. Recently doctor has suggested to take lafarze medicine for 3 months (glucosamine sulphate 2kcl and chondroitin) to repair cartilage injury. Is glucosamine and chondroitin works? Can exercise cure me completely?

I have not seen your MRI . If a cartilage charting shows there is a defect on the cartilage then an arthroscopy is better to treat that defect. Depending on the size of the defect the treatment is biased. The pain is only immediate. If a definite defect is there then it needs to be treated otherwise will lead to early osteoarthritis

All About Acne Vulgaris

Acne vulgaris is a common chronic skin disease involving blockage and/or inflammation of pilosebaceous units (hair follicles and their accompanying sebaceous gland). Acne can present as noninflammatory lesions, inflammatory lesions, or a mixture of both, affecting mostly the face but also the back and chest.

Acne vulgaris has a multifactorial pathogenesis, of which the key factor is genetics. Acne develops as a result of an interplay of the following four factors: (1) follicular epidermal hyperproliferation with subsequent plugging of the follicle, (2) excess sebum production, (3) the presence and activity of the commensal bacteria Propionibacterium acnes, and (4) inflammation.

Acne occurs through the interplay of 4 major factors:

  • Excess sebum production
  • Follicular plugging with sebum and keratinocytes
  • Colonization of follicles by Propionibacterium acnes (a normal human anaerobe)
  • Release of multiple inflammatory mediators

The most common trigger is

  • Puberty

During puberty, surges in androgen stimulate sebum production and hyperproliferation of keratinocytes.

Other triggers include

  • Hormonal changes that occur with pregnancy or the menstrual cycle
  • Occlusive cosmetics, cleansers, lotions, and clothing
  • High humidity and sweating
  • Associations between acne exacerbation and diet, inadequate face washing, masturbation, and sex are unfounded. Some studies suggest a possible association with milk products and high-glycemic diets. Acne may abate in summer months because of sunlight’s anti-inflammatory effects. Proposed associations between acne and hyperinsulinism require further investigation. Some drugs and chemicals (eg, corticosteroids, lithium, phenytoin, isoniazid) worsen acne or cause acneiform eruptions.
  • Acne results in a variety of lesions. The most common acne locations include the face, neck, chest, and back, where the most sebaceous glands are located. Along the jaw line is a common location in adults. "Blackheads" (open comedones) and "whiteheads" (closed comedones) are follicular plugs that are either sitting below the skin surface (whitehead) or oxidized from being exposed to the air (blackhead). Papules are small pink to reddish-brown bumps, pustules are pus-filled lesions, and nodules or cysts are deeper pus-filled lesions.
  • Mild acne consists of a few papules/pustules and/or comedones. Moderate acne has an increased number of lesions. Severe acne has numerous comedones, papules, pustules, and may have painful nodules.
  • Acne can result in permanent scars, which can appear to be depressions in the skin or hyperpigmentation, which is dark red or brown flat marks where the acne lesions were.

Treatment

  • Comedones: Topical tretinoin
  • Mild inflammatory acne: Topical retinoid alone or with a topical antibiotic, benzoyl peroxide, or both
  • Moderate acne: Oral antibiotic plus topical therapy as for mild acne
  • Severe acne: Oral isotretinoin
  • Cystic acne: Intralesional triamcinolone

Starving vs Eating healthy

You study one night before the exam but you manage an all nighter, thus you manage to pass. You study for a week for your sat exams but you manage a score little over 1800. You sit down for a few hours and you manage to finish the work your boss asked for a week ago. You push yourself and get the desired results. These results keep you happy, get the work done (in a long term successful manner) and turn out to be fast as well as effective! but, can you say the same in relation to starving to lose weight? yes it's fast, but is it effective? yes you might look thin, but internally are you happy when you're hungry? yes it's got the job done but will it be a long term success? the answer is no. Starving yourself to lose weight isn't the solution. Why, you ask? well here's a piece of my mind.

Metabolism vs metabolism:
Well the first ones an example of how slow your metabolic rate will be when you starve! since glucose and insulin play a major role in your metabolism levels, insufficient consumption of food will lead to a fall in their levels, eventually causing a decline in your metabolic rate. Then comes the second kind, that fast paced metabolism that's because regular meals keep your digestive system running which in turn paces up your metabolic rates.

No water vs no water weight:
When you decide to starve yourself, you say no to almost any item going into your mouth. You cant live on a peanut for lunch and one fruit for dinner! instead you need to trim the excess weight by getting rid of the water weight by cutting your carbs! for every gram of carb that you consume, your body retains 4 grams of water. No carbs, no retention. No water weight? that's right! say no to carbs, yes to healthy food and bye bye to water weight.

Malnutrition vs nutrition:
After a few months of starvation, there is a major cut in your staple food which leads to insufficient nutrients, vitamin and minerals. Without all the essentials for a balanced diet your body cannot function properly which could lead to serious illnesses and problems. On the other hand, if you eat right your body improves its immunity and keeps even the smallest cough away.

To sleep or not to sleep:
Those who starve themselves tend to get hungry (well, duh) during the odd hours of the night. This either keeps them up late at night or keeps waking them up every few hours. Or, even worse you binge eat at like 3 am. Carbs, fat and dessert! whereas those who eat a well balanced healthy meal end up getting a good night's rest since their body isn't short of food aka energy deprived!

Lethargic lump vs energizer bunny:
After you skip some meals, your body is really low on energy so it uses stored fat as a replacement ie muscles. You'll notice your muscles slowly start to fade away to compensate for the lack of energy in your body. Eventually you won't have the energy to do anything, not even eat (not as good as it sounds!). Actually no energy and no muscle might just make you fat and lazy. If you decide to get off your lazy lump and cook breakfast, you will actually help kick start your body into fat burning mode! also you'll have the energy to exercise which definitely provides better results than starving.

Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Hams Stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. repeat 10 times, twice a day. Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the knee against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.

Eat lot of fruits & green leafy vegetables & drink plenty of water. We need more investigations with clinical examination to decide upon treatment. You may need coronoplasty (smoothen teeth edges) along with t. Rebagen 10, morning one tab & one at night for 5 days, c. Becosules 5 cap, for five days in the morning after meals. Hexigel ointment on the area of ulcer. Rinse your mouth thoroughly with a mouth wash after every meals. Dental tips: - visit a dentist every six months for cleaning and a thorough dental check-up. Limit sugary food to avoid tooth decay. Gargle your mouth thoroughly after every meal. Scrub gently to clean your tongue with a tongue cleaner. A balance diet with minimum 3-4 litres of water. Intake diet rich in fibres, vitamins and proteins. Also have regular visit to dentist. Only diet dosent help you need maintenance of teeth too.
