How long can I take medicine for cholesterol? What are the side effects? Is long-term use harmful?
Cholesterol medicine is given to patients who have risk of heart disease or known case of heart disease. In case of young person like you its not needed unless you have very high level of cholesterol. In either case one has to continue it for lifelong. Yes, there are certain side effects, but the risk is minimal.

What Are Blockages in Heart Arteries

MBBS, MD - Medicine, DM - Cardiology
Cardiologist, Gurgaon
What Are Blockages in Heart Arteries

Blockage in heart is a common term used for narrowing of coronary arteries. Coronary arteries are vessels, which supply blood and thus oxygen and food to continuously working heart muscles. Heart muscles, which are not tired working from the birth till death, however, cannot sustain long without blood supply.

A reduction in blood supply gives rise to ischemia of heart muscles commonly manifested as chest discomfort or angina. A sudden complete shutdown of blood supply leads to heart attack leading to permanent damage to heart (if blood flow not reestablished promptly).

But what causes these arteries to block? Deposition of LDL cholesterol (low density cholesterol) in inner surface of coronary arteries is the primary reason of these blockages. LDL a normal component of blood (upto certain limit) starts depositing in arteries as early as 10 years of age!

Deposition of billions of LDL molecules over several years on inner surface of arteries gives rise to visible narrowings in these arteries. Flow ahead of these narrowings is reduced in proportion to the narrowing. At a level of 70 % narrowing the flow is reduced to give ischemia (and angina) during exercise. Gradually increasing degree of narrowing reduces the exercise needed for ischemia and angina; a narrowing of more than 90 % can give symptoms at rest. A sudden clot formation at any of these stages can block the flow suddenly giving a heart attack.

If LDL is a normal component of blood, why it is deposited in the arteries at first place?
LDL above a certain limit in blood starts depositing in the arteries. Diabetes, Hypertension, smoking, less exercise and genetics makes it more sticky thus making narrowing faster. This is why these risk elements need to be properly attended for prevention from heart disease. For treatment medicines are important for stopping the progression of narrowings; angioplasty is a method of fast resolution of blockage; and bypass surgery is the method of creating a whole new blood supply for the affected part of the heart.

Sleep Deprivation Can Cause High Blood Pressure

C.S.C, D.C.H, M.B.B.S
General Physician,
Sleep Deprivation Can Cause High Blood Pressure
Is Sleep Linked to Hypertension?

The link between sleep and hypertension is well-known. Studies have examined the effects of sleep deprivation on healthy volunteers and have examined the sleep patterns of people with hypertension, producing data that suggest adequate sleep may reduce risk. good cardiovascular health. Data from several studies show that people who sleep less than six hours each night are 20% more likely to develop high blood pressure.
One night of inadequate sleep in patients with hypertension has been shown to result in elevated blood pressure throughout the next day.

Average sleep times have declined

Our modern society runs 24 hours a day, and many of us curtail sleep time to keep up. From an average sleep duration of 8 to 9 hours in 1960, our national sleep duration has dropped to 6.9 to 7 hours. Many people try to get by on five to six hours of sleep nightly, a habit that may be contributing to serious long-term health conditions.

What happens when you sleep?

Sleep is restorative, most people agree. We usually don’t question why, but the fact is that the circadian rhythms of sleep regulate our nervous system and the hypothalamic-pituitary-adrenal axis, two critical body systems that keep our bodies in healthy balance. During sleep, normal people should experience a drop in blood pressure of about 15 points, on average. This reduces the work of your heart.

The Autonomic Nervous System and “Fight or Flight” Response

Sleep regulates the autonomic nervous system, that part of the nervous system that modulates the “fight or flight response.” This evolutionary response causes changes in many bodily functions that at one time were useful to provide an edge against predators.A constant state of preparedness from inappropriate activation of this response results in harmful stress on the body.

When the sympathetic nervous system is stimulated, blood vessels are constricted to deliver blood to vital organs like the brain and heart, increasing blood pressure. The “fight or flight” response is also associated with changes in glucose metabolism and an increased risk of insulin-resistant diabetes.

Cortisol, Adrenaline, and the Hypothalamic-Pituitary Axis

The primary hormones regulated by the pituitary and hypothalamus during sleep are adrenaline and cortisol, released by the adrenal gland. Adrenaline is a potent hormone that has a direct effect on blood pressure, mediated by constriction of the arteries. When your adrenaline levels remain high during the night, it can result in sustained hypertension.

Cortisol is a “stress hormone” that is highest in the morning and reaches a nadir between midnight and four AM. Lack of sleep can result in significant disruption of the cycle, subjecting your body to unnecessary stress responses and fatigue that are as damaging to your health as poor diet or lack of exercise.

When you awaken in the morning, your body typically experiences a 50% rise in cortisol level as your body prepares for the stress of a new day. Studies show that waking up early in the morning increases the cortisol response, an effect that is pronounced in people who are facing chronic stress and worry. Cortisol levels usually decline throughout the day, but in people who suffer from sleep loss, cortisol levels increase in the early evening, preventing natural recovery from the day and preparation for a restful night. In addition to lowered immunity, impaired glucose tolerance, and increased craving for carbohydrates, sleep deprivation is associated with elevated estrogen levels, decreased alertness, and poor concentration.

Sleep and Thyroid Hormone

Sleep loss also increases the amount of thyroid hormone in people who are not getting enough rest. People with elevated thyroid hormone have both increased blood pressure and cardiac output, putting unneeded stress on the heart.

Sleep, Obesity, and Hypertension

Sleep deprivation increases appetite by disturbing the regulation of leptin and ghrelin, two hormones that modulate appetite. Sleep deprivation alters your body’s ability to regulate the need for calories, resulting in over-eating and obesity, also linked to increased risk of hypertension.

Caffeine and Hypertension

Many of us use caffeine to remain alert when we haven’t slept well, a habit that causes dramatic increases in blood pressure. The mechanism for elevation of blood pressure after drinking a caffeinated beverage is not completely understood. Some researchers think caffeine may stimulate the adrenal gland to release adrenaline, a hormone with direct effects on blood pressure. It may block hormones that keep the arteries relaxed.

Obstructive Sleep Apnea

People who have obstructive sleep apnea (OSA) have multiple episodes during the night in which they stop and restart breathing. This disorder usually affects middle-age to older adults, but it can occur in patients of any age, particularly people who are overweight. People with OSA typically have high blood pressure, particularly on awakening, when their blood pressure should be at its lowest point. Symptoms of OSA include daytime sleepiness, loud snoring, morning headache, and difficulty concentrating during the day. They may be observed gasping suddenly during the night before returning to sleep.

Are You Getting Enough Sleep?

Sleep is critical for maintenance of your health. If you work shifts or curtail your sleep to accomplish multiple tasks, you are at risk for hypertension that can be difficult to treat. Measure your blood pressure in the morning. It should be at its lowest level and if it’s elevated, you should see your doctor in addition to consideration of lifestyle changes to prevent progression of medical problems associated with hypertension and sleeplessness. If you have symptoms of obstructive sleep apnea, there are effective treatments available.

Hi, I'm 4 months pregnant, I am having headache everyday from last week. Which make me uncomfortable. Please give me ...
Hello- headaches are triggered by the hormonal changes that are happening in your body. They're very common in early pregnancy. The hormone progesterone helps to relax your womb (uterus) and blood vessels in your body, including those in your head and neck. Blood then pounds through these veins, causing headaches. Try these tips to avoid headache during pregnancy- 1. Rest and relaxation get plenty of rest and maybe take time off work, if you can. Get an early night every now and then. 2. Eating well try to eat a healthy diet. Have a wide variety of unprocessed foods of all types and colours to make sure you're getting a good balance of nutrients. 3. Posture make sure the position of your chair, computer screen and car seat encourage good posture.

There are many reasons which may cause blood pressure to lower down, it can be anemia due to blood loss, low fluid intake, low salt intake, hypothyroidism etc. Firstly increase salt in diet, drink lots of water, coconut water is good for low blood pressure. If still it persists after following this, then revert.

High cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take omega-3 fatty meal or cap., take .green veg., whole grain.garlic,. Take anti cholesterol medicines .

I have chest pain since 10 months my ECG report and chest x Ray is normal now days my right hand ring finger is pain ...
More likely to be cervical neuralgia due to pinching of nerves in cervical vertebrae. Get one x ray cervical spine to confirm. Syop using pillowsbelow the head, dont work with forward bending of neck for long. Do back bending exercises of neck. You should be relieved of the pain. You may not need Stress ECG unless your pain starts only after some exertion like walking, running etc.

Dear lybrate user, I think you feel chocked. There are lot many things going on in your mind but you are unable to express it. You might be scared and probably feeling shameful about something in life. You might be angry with someone or something. You need to express your feelings which will relieve your anxiety and stress. Once you are relieved of your troublesome thoughts and feelings, you will start feeling lighter. You will start taking interest in life and may have more appetite and eventually put on more weight. If there are any problems at home, try to discuss with concerned people and try to resolve them. You may talk to your school or college counsellor. The counsellor will help you. Take care.
