I am suffering from jaundice and I m not able to digest anything, so what food items are safe for me. Does jaundice s...
Jaundice is yellowish discolouration of the white part of the eyes. It occurs because of raised yellowish pigment, bilirubin in the blood circulation. Bilirubin in circulation rises due to various causes one of the common causes we know are related to hepatitis which commonly occurs due to viral infection. Apart from that there are various other causes which are responsible for elevated circulating bilirubin. These causes me include gallstones stuck in the bile duct, cancers causing obstruction to bile duct or occupying big chunk of liver to cause it to dysfunction. Excessive destruction of red blood cells may cause a type of jaundice. Treatment of the jaundice depends on the cause for it. Broadly patient with jaundice if they are non diabetic, they are advised to take high glucose diet.

Heart Transplant - When Is It That You Have To Go For It?

MCH DNB (CTVS), Advanced fellowship, MS
Cardiologist, Delhi
Heart Transplant - When Is It That You Have To Go For It?

There are many people worldwide who suffer from heart problems (irrespective of their age and sex). While in most cases, the condition improves with proper treatment and medications, in few, the condition is beyond treatment. A heart transplant comes as a savior for such people. It gives them a new lease of life. The transplant involves replacing a heart that has stopped functioning normally (damaged or may be diseased) with a healthy heart (from the donor).

Over the years, heart transplant has undergone a sea of change. With the advancement of science and technology, the success rate in a heart transplant has seen an exponential rise.

People who need a heart transplant:
A heart transplant may be essential in the following cases.

  • A congenital heart disorder (a person born with a heart problem).
  • Defective or diseased heart valves.
  • Amyloidosis (a condition where amyloid fibrils get deposed in the tissues and organs of the body intracellularly or extracellularly).
  • Problems in the coronary artery.
  • Cardiomyopathy (A condition where the muscles of the heart become weak, thereby affecting the normal functioning of the heart).
  • A heart transplant that failed previously.
  • Ventricular Arrhythmias (a condition that originates in the ventricles, in ventricular arrhythmias, the heart rhythms are abnormally rapid).

However, under the following circumstances, a heart transplant may not be a wise idea

  • People with infections or chronic lung or kidney disorders.
  • A case of cancer in the past.
  • Age may be a deciding factor. The recovery from a heart transplant may not be 100% in an aged person.

The heart transplant procedure:
The first step in heart transplant is the availability of a suitable donor. In this case, a donor is a person whose brain is dead but the other organs, including the heart, is healthy and functioning properly. A surgeon performs three operations in a heart transplant.

  • The first operation is essentially the removal of the healthy heart from the donor body. The heart is kept in a cool place, preferably ice (to keep the heart alive and in good condition until the heart transplant takes place).
  • In the second operation, the recipient's damaged or diseased heart is operated out.The situation may, however, be complicated if the patient had a heart surgery in the past.
  • The third and the final surgery involves implanting the donor heart into the recipient body (the recipient's upper heart chambers and the atrial back wall are however not removed).
  • Once the implantation takes place (without any complications), the surgeons sew the heart into place.
  • The blood vessels are then connected back to the heart and the lungs. The heart starts beating again once it is warmed up.
  • To enable the patient to receive the nutrients and oxygen (during the heart transplant), the patient is put on a heart-lung machine.
  • If no complications develop after the transplant, the patient is discharged within a fortnight.

In some unfortunate cases, there may be organ rejection. The condition arises when the recipient's immune cells see the transplanted heart as non-self (foreign agents). If left unattended, it may damage the heart. Immunosuppressant drugs can help avert the rejection. However, it is important to monitor the patient closely for any infections that may arise to the administration of the immunosuppressants. If you wish to discuss about any specific problem, you can consult a Cardiologist.

Lipid profiling since 20 can prevent heart ailments: doctors
Lipid profiling -- blood tests that screen for abnormalities in lipids, such as cholesterol and triglycerides from the age of 20 years can help detect heart ailments at an early stage.

Lipid profiling -- blood tests that screen for abnormalities in lipids, such as cholesterol and triglycerides from the age of 20 years can help detect heart ailments at an early stage, revealed doctors on thursday.

According to them, a person may be at high risk of heart attack if the levels of high-density lipoproteins (hdl), low-density lipoprotein (ldl) and triglycerides -- causes of heart ailments -- are high.

These should be screened early, especially if the person is a smoker, obese, has blood sugar level on the higher side or there is a family history of heart ailments.

This blood test (lipid profiling) indicates results of the levels of hdl, ldl and triglycerides. An annual frequency of the test is recommended for people with diabetes or heart issues.

Triglycerides, a type of fat, if seen at elevated level, has a strong association with metabolic syndrome which has a tendency to increase the risk for heart diseases, diabetes and stroke.

According to the world health organization, cardiovascular diseases would be the largest cause of death and disability in india by 2020. By 2030, it is expected that 23 million people will die from cardiovascular diseases annually.

Medical science says that lipid profile also helps in diagnosis and making decisions about what treatment would be best if there is borderline or high risk, if found positive for heart ailments. Monitoring and maintaining healthy levels of these lipids is important in staying healthy.

Chabbra urged people to maintain the body mass index (bmi) within the normal range (19 to 25), as it could determine the risk for developing obesity, another cause for heart ailments.

According to the world heart federation, at least 80 per cent of premature deaths from heart diseases and stroke could be avoided if the main risk factors  tobacco, unhealthy diet and physical inactivity are controlled.

Suggest what to do?

Reparenting Technique, BA, BEd
Psychologist, Bangalore
If you act fast and responsibly, your daughter may be able to recover fast and with good results. The symptoms coming at this time is typical of issues not yet resolved from her childhood. She should immediately meet with a counselor and talk about these matters. I request you and your wife to accompany her and inform the counselor of her entire history from prenatal days, and a brief of her family background from up to her grandparents on both sides. In the meantime she should continue on the medication, unless you want a second opinion, and then proceed with the medicines prescribed. Whatever happens please do not discontinue her therapy or medication without their advice. The counseling needs to be there right up to her recovery. If you know of anything in her childhood that might have contributed to her condition you must reveal that to the doctor. It will help them come to a proper diagnosis. When you mention? mood swings, there is a more definitive interpretation to be pursued.

Hi while having sex I can able get mood for oly few min. Is it affect pregnancies.

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
Hi while having sex I can able get mood for oly few min. Is it affect pregnancies.
Failing to do Foreplay can ruin mood or leave you unsatisfied. Sex without foreplay means you’re not taking your needs into consideration, because foreplay is much more important to women than to men. Foreplay is a set of emotionally and physically intimate acts between couple meant to create sexual arousal and desire for sexual activity. Vagina secretes natural lubricating solution which is secreted by bartholin's gland. Foreplay also helps in secreting natural lubricant which will improve sex life.
