As long as there are no pregnancy related issues like bleeding or spotting noticed or her doctor has contraindicated, it is alright to have sex in pregnancy.

Ways of Dealing with Intestinal Distress

MS - Surgery, MAMS, Senior Residency , MBBS, Fellow IAGES

Ways of Dealing with Intestinal Distress

Undigested food that lurks around in the digestive tract produces a host of disorders due to intestinal distress. These disorders range from acidity to gastro-oesophageal reflux and even colon diseases like cancer, irritable bowel syndrome, constipation, gallstones, ulcers and Crohn's disease. Cramps, nausea, vomiting, loose motions, gas, and acidity are some of the most common symptoms that visit the patient as a result of such ailments. There are many ways of dealing with such intestinal distress, as follows :-

  1. Diet: Introduce plenty of fibre into your diet to aid the digestion process. This can cut the risk of many such ailments and also aid in the treatment of the same. Intestinal distress is largely caused by food that is still lurking around in the digestive tract, so it is important to clean out the digestive system with plenty of fibre and water.
  2. Nutrition for better Digestive System Health: Food that is rich in minerals, vitamins, antioxidants and iron will help in creating a stronger system that cleanses the digestive tract and helps in reducing the production of excess acid in the stomach. A balanced diet with plenty of nutrition aids digestion and makes the immunity stronger.
  3. Study your Triggers: Watch out for foods that can trigger reflux action and other acidity related issues on the intake. These can include fatty food, fried and oily food as well as extremely spicy food. Stress is also a major trigger of intestinal distress, so it would be a good idea to avoid stressful situations or take medical help to deal with them in a better manner.
  4. Medication: Antacids and other medication to prevent the onset and growth of such distress can be sought out after due consultation with a medical practitioner. Medication will help in dealing with and altering the acid level production in the stomach.
  5. Surgery: In very extreme cases like ulcers and large size gallstones, surgery may be helpful so that you avoid rupturing the stomach's lining which can lead to a serious infection or even internal bleeding.

Knowing your triggers and getting into a better routine as far as your diet goes is as important as seeing a doctor about the severity of your case when it comes to intestinal distress.

If the girl had regular normal periods, it is less likely to be a pregnancy. (exception is an ectopic pregnancy in which case the female may get spotting instead of the regular periods)

The chance of a pregnancy per cycle is 20-30% however in your case wud be reduced in alternate months. If you are that worried see a gynaec who can run some tests and decide on whether you need further medication to help you with pregnancy.

Yes, very true, you should get a urine pregnancy test done and at the same time an ultrasound lower abdomen to see if all is well. And also avoid taking self medications or I pills on a regular basis because it also has its own effects on harmonal cycles.

If you are having normal periods every month then it is unlikely you are pregnant. However you can get a urine pregnancy test done if in doubt. If the discharge happens throughout the month and it is bad smelling or associated with itching you will need a course of antibiotics.

What Is an Insufficient Cervix and How Can It Be Treated?

MICOG, MS - Obstetrics and Gynaecology, MBBS
Gynaecologist, Gurgaon
What Is an Insufficient Cervix and How Can It Be Treated?

The cervix is a part of the female reproductive system. It is the lower part of the woman's uterus, and it connects the vagina with the uterus. This is usually closed and tight, but ends up dilating during occasions like pregnancy and childbirth. The cervix is also known to soften eventually during the course of one's pregnancy and delivery. In case the cervix starts to open up too early, this condition is known as an insufficient cervix or an incompetent cervix. Here is everything you need to know about this condition.

Definition and symptoms: An incompetent or insufficient cervix is one that opens up too early and does not support a normal delivery. The symptoms that may point at this condition include pressure in the pelvic area, pain in the back, light bleeding from the vagina, mild cramps in the abdomen, and a change in the odour and hue of the vaginal discharge that may be experienced by the pregnant woman. Usually, such symptoms and changes are not seen in the early stages of one's pregnancy. Mostly, the symptoms are experienced after the fourth or fifth month of pregnancy, in the second trimester, when there is greater pressure on the area due to the growing fetus.

Risk factors: There are a number of factors that increase the risk of this condition, including congenital conditions like uterine anomalies and other genetic disorders. These disorders may affect the fibrous protein that produces the connective tissue of the body, which is also known as collagen. When there is a decrease in this kind of tissue, the cervix becomes incompetent or insufficient. Also, when there has been overexposure to synthetic estrogen, this kind of condition may be experienced. Further, cervical trauma is another factor that may cause this condition along with dilation and curettage, which is a procedure that corrects uterine conditions.

Treatment: The main form of treatment usually includes the prescription of progesterone supplements in the form of weekly administered injections. Also, serial ultrasounds every fortnight can help in monitoring the situation closely so that the doctor is able to ascertain whether or not you are going to be able to go through a normal delivery or not. Cervical cerclage is also a surgical procedure that can help such a condition. In this procedure, the cervix is stitched up with strong sutures, which will be removed in the last month of pregnancy, or just before the delivery. This is especially helpful if there have been cases of premature delivery in the past. This procedure is usually carried out within the first 14 weeks of the pregnancy to avoid loss of the fetus.

Self Monitoring of Blood Glucose - SMBG Chart

MBBS, Diploma in Diabetology
Endocrinologist, Mumbai
Self Monitoring of Blood Glucose - SMBG Chart

Self-monitoring of blood glucose (smbg) is very crucial for those diabetics who are on insulin or on certain oral anti-diabetic drugs which can cause hypoglycemia. To achieve best glycemic control it is essential to know how your daily blood glucose levels are fluctuating in an entire day.

It is easier for a diabetes specialist to adjust drugs or to adjust insulin dose if good smbg record is maintained by the patient.

Normally we check fasting and 2 hr post prandial blood sugar levels in the lab for routine follow-up. There is a bright chance of sugar excursions in the other post meal readings which get unnoticed. Even in case of hba1c or glycated hemoglobin test we get an average result of blood glucose of last 3 months.

In practice, I have seen several patients who had to fast and post prandial blood glucose lab tests in the normal range but hba1c levels below 7 is not achieved because they might have high blood sugars in other post meals which have got unnoticed.

To achieve the best blood glycemic control it is important to control blood sugar levels within normal limits during the entire day and not just fasting or post breakfast levels.

Interpretation of smbg chart:

In the above image of smbg chart, I have mentioned 7 point charting system where you can fill your blood glucometer readings which you take on several spots in an entire day.

Seven points includes:

    Fasting or before breakfast reading
    2 hrs after breakfast
    Before lunch
    2 hrs after lunch
    Before dinner
2 hrs after dinner or bedtime
    At mid night 3.00 am

This seven point smbg charting approximately covers entire day glucose fluctuations to some extent (of course not as accurate as cgms).

I have also mentioned a comment column in the above smbg chart where you can note down change in food or change in insulin dose or any other changes that you have made which probably caused unusual readings like for example if hypoglycemia occurs at before lunch reading and you have skipped breakfast or had very little snack at breakfast you can mention that.

You do not need to check daily all the 7 points, you can check any one or 2 points randomly depending upon your sugar control.

More uncontrolled blood sugars require more frequent smbg glucometer monitoring.

Monitoring of blood glucose at home in case of diabetic pregnancy or gestational diabetes is also important where frequent monitoring and smbg charting of glucometer readings helps a lot to the patient.

I hope this article will help you to control your blood sugar levels more efficiently.

If you find that some of the post-meal or pre-meal readings are high or low, please consult your diabetologist with smbg chart to adjust your medication or insulin dosage.

If you like my article please share it with your friends and family.

Awareness is prevention!

Bleeding during pregnancy can occur because of some little bit detachment of placenta if both you and your baby are fine now then don't worry just follow your doctor's advise and relax.

Music - an Antidote for Stress
Music for Stress Management

There are times when you are either too stressed or too pained to feel good about anything. Stress from hectic work schedule and daily life gets you and makes you prone to health issues or feeling depressed and lonely. The resultant extreme mood swings take over your life and you are no longer in control!

One very effective and interesting way to beat the stress is to switch on some Music!

Research has proven that soothing music such as classical instrumental pieces seem ‘melodious’ to us because of the calming wavelengths that they resonate at. These sound waves produce a sense of harmony and well-being in our minds. As we listen, these sound waves ease out our ‘stress’ and relax the nervous system. Stress is your worst enemy and the only way to live life is to enjoy every moment of it!
