Sperm counts between 20 and 30 million/ml of semen are ideal for sperm washing and subsequent IUI, while success rates are lower if the prewash sperm count is less than 10 million/ml of semen. I néed to know more details to guide you further.

I was was born in 1988. I was suffering from asthmatic problem when I was three years old. Suddenly after two years the problem disappeared. Got married in 2005. Since then I am suffering from allergies of dust, food and contact to dirtiness. When I suffer from allergies, I get angry very soon. Most of the time I spend on irritation. I want to get rid of allergies as well as irritation.

MS - ENT,MBBS
ENT Specialist, Kolkata
Dust allergy is partly genetic and partly environmental. Thus it never goes away. With better, less dusty environment  the extent if the problem gets reduced. Medicines help to downregulate the immune response of your body to these environmental allergens. Long term lifestyle  changes with gradually tapering medication  helps to tackle chronic sinus problems.  Start taking Steam inhalation twice a day. *For chronic allergic rhinosinusitis you will need to take a long term course of antibiotics  together  with Nasal sprays. *You will need to avoid dusty environment  and dust mites, animal dander etc. Regular cleaning of AC air filters. Wearing mask while driving 2 wheelers/working in dusty  areas Medical  management  is done for 3 months and then patients  condition is reviewed.  Intermittent treatment is not beneficial  in the long run. Around 30 %of people with allergic rhinitis  will develop Asthma if not treated. But majority of patients with asthma are allergic to dust. Active treatment  of nasal allergy reduces Asthma attacks and medicine requirements. This improve  quality of life. Hope this helps

Hello, I am having pin pointing pain over my left chest. I have consulted with cardiologist and get TMT and echo test and everything is fine. I am having this pain from almost an year now. Recently a month ago I noticed than my left ribcage has come little outwards from the center and also one of the bone of my left ribcage has come out a little (below left underarm). I think pain is starting between these points where rib bones are disordered. I do workout daily but due to this pain I have stopped lifting heavy weights from last 2 months. Is there anything to worry about rib bones disorder, please share your view it would be really helpful for me. Thanks Sincere regards, Harshit.

Hello, I am having pin pointing pain over my left chest. I have consulted with cardiologist and get TMT and echo test...
Dear lybrateuser, - Since you have pain when doing workout & you are able to pinpoint the location it is due to bone problem & the surrounding tissues - take rest do not aggravate it by exercising & lifting weight - apply pain killer gel three times a day on the affected area, apply lightly do not rub - take a nsaid tablet like brufen if pain is more - get x- chest done to find out exact cause & consult a doctor.

I can suggest eating at regular intervals. Eat less but frequently, if you are bed ridden. Drink water either half hour before food or 2 hours after food to prevent reflux. Avoid fried and spicy foods. Eat plenty of greens.

Nutritional Concerns During Menopause!

Nutritional Concerns During Menopause!

Menopause is a phase when the estrogen in the body starts to diminish. Every woman has to face this "change of life" at the time of her last period. On average, women reach menopause at age 51, but it can happen earlier or later. Menopausal symptoms vary with every woman. Common symptoms include hot flashes, night sweats, weight gain around the middle, sleep disturbances and mood changes. However, some women go through menopause with no real symptoms.

What causes menopause? Hormones. As women age, your ovaries produce less estrogen and progesterone, two of the main hormones for reproduction. As estrogen levels go down, one of the first signs of "menopausal transition" is irregular periods in which bleeding is unusually heavy or light; the time between periods also may become longer.

While the symptoms and the risk factors of menopause cannot be negated in totality, good nutrition can ease certain condition for the body. A variety of food along consisting of enough iron and calcium is of prime importance during this phase. Here is a basic guideline of the dietary her healthy practices one should follow:

  1. Enough calcium intake: A daily serving of three to four dairy based products can pay rich dividends in the endeavor of acquiring calcium for the body. Other calcium-rich food sources include fish such as salmon and sardine, legumes and broccoli. The aim should be to intake a minimum of 1200 mg of calcium daily.
  2. Iron is a must: Three servings of iron-rich diet is a necessity for the body. Some iron rich food sources include red meat, nuts, grain products, green vegetables, red meat, poultry etc. Acquiring 8 MG of iron daily is a must for every woman going through the phase of menopause.
  3. Fiber is a key dietary component: The intake of fiber rich food is a necessity during menopause. Some fiber-rich food includes bread, fresh fruits, pasta, cereals etc. A daily intake of 21 gms of fiber content is essential for the body to function properly.
  4. Gamma Linolenic Acid: This is a form of fatty acid that is extremely crucial to regulate the balance of hormone and maintain health during menopause. Good sources of Gamma Linolenic Acid or GLA are vegetable oils originated from hemp seed, primrose, blackcurrant etc. Intake of these oils along with curry or salad should do the trick.
  5. Intake of Magnesium: The human skeleton is comprised of 60 percent of magnesium. Over 300 enzymes require magnesium for a various catalytic reaction of the body. Needless to mention that intake of magnesium is crucial for a woman going through menopause. Some good sources of magnesium include halibut fish, leafy vegetables, grains, seeds etc.
  6. Intake of Phytoestrogens: Soy isoflavones are rich in phytoestrogens. Good source of the same include soya milk, soya beans, tofu etc. Soy protein is extremely beneficial due to their support function of HDL. It can even influence positive heart function. Studies have found that phytoestrogens can successfully limit the flow of free radicals in the body and block the signs of aging.
  7. Antioxidants Vitamins: Vitamins, especially A, C, and E are known to be mighty effective in maintaining good menopause health. Food items such as kale, butter, turkey, pork are a great source of vitamin A. Rose hips, lemon, strawberries, sprouts are good sources of vitamin C. Vitamin E can be found in certain food items such as avocado, spinach, hazelnuts, sunflower, milk, asparagus, whole grain etc. If you wish to discuss about any specific problem, you can consult a Dietitian/Nutritionist.

I have a problem in my penis. It is too large but not so fat. Is dat any kind of problem? What should I do now?

BHMS
Homeopath,
I have a problem in my penis. It is too large but not so fat. Is dat any kind of problem? What should I do now?
Dear lybrate user, the size of penis depends on four factors:- 1) height of the person:- every organ of a taller person posses greater dimension than those of a shorter person. 2) testosterone hormone:- less secretion of this hormone leads to underdevelopment of sexual organs 3) sexual diseases:- patients suffering from erectile dysfunction often complaints of short penis. 4) fatty tummy:- with every increase of 6 kg in weight, the waistline is increased up to 2cm. This fatty layer of abdomen conceals 2cm of the length of penis by surrounding it. The minimum size of an erected penis required for intercourse is 3 inches. A penis is composed of three highly vascular muscles. The plan of action behind increasing the size of penis is to increase the amount of blood flow to those vascular muscles of penis. There is no specific medicine to increase the size of penis. To increase the size of your penis you can try the vacuum operated penis developer set available in market. You can buy it from local medical appliance store or you can order for it online. Use the penis developer set for at least 30 minutes daily. For better & quicker result, apply ayurvedic himcolin gel on your penis before using the penis developer set every time. Kindly remember that the penis will not remain erected all time. It will remain contracted & flaccid throughout the day. However, you can observe the increase in the width & length of your penis whenever you undergo erection.

You must go and meet with a counselor immediately and if that person advises that you meet with a doctor you must do so and cooperate to your utmost. Please visit these professionals along with your parents. In the meantime please do the following sincerely because you could resolve the problem better with good cooperation: Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber, nuts, avocado, exercise regularly, eat dark chocolate, do Yoga meditation exercises, etc. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend Yoga classes etc. Watch sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least, 30 minutes but not in the scorching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love. Learn all about emotions and how to handle them and that will get you out of the depression rather easily and quickly. A counselor is there only to facilitate you, all the hard word must come from you, and your cooperation with that person is very critical for your success. Be positive every day and learn to be contented with what you have. Do some left brain exercises: it is the happy brain. Here are a few suggestions: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do callisthenic exercises with some form of counting, regularly. Whatever happens please cooperate with the therapy and do not discontinue until the condition is completely resolved.

Sleepwalking: What You Should Know About It

DPM
Psychiatrist, Mumbai
Sleepwalking: What You Should Know About It

Sleepwalking is also known as somnambulism is a type of  parasomnia. A parasomnia includes undesired interruption in ones sleep.  Sleepwalking happens when you get up from your sleep, and walk around your house. You are actually still in a state of sleep when you are sleepwalking around. It can, likewise, include a progression of other complex activities. You are likely to suddenly get up from sleep and sit on your bed, looking around like you are dazed or confused. Another type of sleepwalking involves abruptly rising in the midst of sleep and running away. Apparently, the patients feel that there is an acute threat, and they must escape from it by running away.

There are more features of sleepwalking. Here is a list of facts on sleepwalking which you should know.

  1. You may talk or yell as you are sleep walking. Your eyes are normally open and have a look of confusion. The eyes also seem to be glassy. You may perform some activities, which are not supposed to be done at night.
  2. Strange, peculiar and weird actions are carried out while sleepwalking, in inappropriate places. Urinating in a garbage can, rearranging your furniture or jumping out of the window are common weird activities, which can be observed in sleepwalkers. The patient turns very violent and arrogant.
  3. In uncommon cases, a patient will get into a car and set out for some unknown location. The patient may even choose to get separate for a long duration. Exposure of inappropriate body parts is also observed in sleepwalkers. Grown ups may dream or fantasize while they sleepwalk.
  4. Some people tend to eat during sleepwalking phase.
  5. Waking up a sleepwalker is a very difficult thing to do. When a patient finally wakes up, he is in a state of utter fix and confusion. No memory of the events that happened during sleepwalking remain, and a patient cannot recall anything. Adult patients may sometimes recall very few traces of what might have happened. A clear memory of the events is rarely found in sleepwalkers.
  6. At times, the patient becomes so violent that he may attack anyone who tries to wake him up. This is seen more in men. The sleepwalking may end abruptly, and the patient will find himself in an unknown place. The last memory that he might recall is going to sleep in bed. Some patients return  to their beds after a stroll automatically, without waking up.
  7. Sleepwalking usually occurs in the first ot the third half of a person's sleeping phase.

Sleepwalking is an unhealthy phenomenon and measures should be taken if you find someone to be sleepwalking.

Hi, there are two types of insomnia: primary insomnia and secondary insomnia. Primary insomnia: primary insomnia means that a person is having sleep problems that are not directly associated with any other health condition or problem. Secondary insomnia: secondary insomnia means that a person is having sleep problems because of something else, such as a health condition (like asthma, depression, arthritis, cancer, or heartburn); pain; medication they are taking; or a substance they are using (like alcohol). Sleep plays a vital role in good health and well-being throughout your life. Getting enough quality sleep at the right times can help protect your mental health, physical health, quality of life, and safety. In children and teens, sleep also helps support growth and development. The damage from sleep deficiency can occur in an instant (such as a car crash), or it can harm you over time. For example, ongoing sleep deficiency can raise your risk for some chronic health problems. It also can affect how well you think, react, work, learn, and get along with others. Management: here are five tips everyone can use to help improve the quality of their sleep: keep your bedroom cool and dark put away/turn off all electronic devices while preparing for bedtime stick to a regular bedtime and wake time every day, even on weekends stop drinking caffeine by the early afternoon and avoid large late-night meals skip the late-afternoon nap, as it can make it harder to sleep at bedtime avoid taking unnecessary stress/ tension. Do regular exercise. Go for a brisk walk in the morning. Medication: take schwabe's bacopa monierri 1x/ thrice daily and kali phos./ once daily at night for 1 month.

And witch medicine I will take that time? Thanks please reply one I'm very upset. I'm a student.

Hello sir.
From last one year I have heart palpitation problem. Yesterday my heartbeats gone very slow for 30 seconds...
1st degree heart blocks only delay the passage of impulse from atria to ventricles and as such there is no loss of beats so you should not read too much in this. As far as palpitations are to be considered, this may be due to your anxiety and over consciousness. Try to keep yourself busy in your routine and diverted from this. In case your PR interval gets prolonged in any subsequent examination then reassessment of cardiac function may be done.
